Our fast growing, post-acute care practice is currently searching for a full-time Nurse Practitioner or Physician Assistant to assist at a skilled nursing facility (SNF) in Santa Clara County. The clinician will provide intermittent coverage for post-acute rehab patients and long-term residents.
We are looking for an NP or PA who can monitor, deliver, and improve upon comprehensive quality patient care in a post-acute and long-term care setting. The successful applicant will have the opportunity to apply a team concept approach to post-acute care delivery. The clinician will work independently, as well as directly with the Medical Director and Medical Coordinator to deliver and coordinate comprehensive care plans.
POSITION SUMMARY:
This is an onsite clinical position where the NP/PA is responsible for providing direct patient care with an excellent support system. The clinician will provide intermittent coverage for post-acute rehab patients and long-term residents while operating in partnership with the Medical Director. The NP/PA will also have support from a dedicated Medical Coordinator. You would see approx. 15-17 patients a day depending on the daily census. Census will be provided daily.
The ideal candidate will be committed to making a real difference in their patients’ lives, continuing their education and strengthening their practice under the guidance of a skilled medical director.
KEY RESPONSIBILITIES/ESSENTIAL JOB FUNCTIONS:
- Evaluate patients from admission and ensure that discharge and rehab plans are executed.
- Monitor for acute changes; diagnose and treat common conditions.
- Propose treatments for chronic and infectious diseases.
- Maintain accurate records and schedules.
- Participate in quality improvement programs.
- Available via phone or text when on call.
- Assist with initiating/documenting Advanced Directives/POLST form.
Daily Visits
- Order diagnostic tests, reviews labs, radiology reports, and consults on all patients.
- Talk to and examine assigned skilled-level patient on daily rounds.
- Assess rehab progress.
- Discuss concerns with the patient, family, rehab, and case management.
- Educate patient and family members regarding acute and chronic illness management.
- Attend family meetings as necessary.
- Inform medical director of significant changes in medical condition.
- Discharge: coordinate transition from skilled to long term placement or home with care coordinators.
Long-Term Care
- Follow long-term patients every 30 days or as acutely needed.
- Perform diagnostic and/or therapeutic procedures within your level of training and expertise.
- Order, interpret and evaluate diagnostic tests for both regularly scheduled health maintenance and acute clinical issues.
REQUIREMENTS/SKILLS:
- Proven experience as an NP or PA in primary care or sub-specialty outpatient clinics, hospitals, or SNFs
- Expertise in primary care/internal medicine/post acute care
- Geriatric background preferred but not required
- Valid NP or PA license in California
- Current CA DEA certificate
- Knowledge of health and patient care regulations
- Strong ethics & excellent communication skills

- Willingness to continue gaining knowledge and clinical experience
Job Type: Full-time
Pay: $150,000.00 - $175,000.00 per year
