NYC Health + Hospitals/Gouverneur (MANHATTAN) provides primary and specialty outpatient care through its Diagnostic and Treatment Center (D&TC) and residential care for the elderly and infirmed in its 295-bed Skilled Nursing Facility (SNF).

Job Description:
-
Under general supervision, directs the development, implementation and evaluation of the nursing regimen for patients/clients within an assigned unit involving the diagnosis and treatment of responses to actual or potential health problems by means of nursing methods and techniques such as case-finding, health teaching, health counseling and the provision of care supportive to and restorative of life and well-being. Schedules, assigns, guides, directs and instructs nursing personnel in nursing activities.

Coordinates, guides, and directs the development, implementation and evaluation of nursing care based on nursing process.

Supervises nursing activities and determines necessary resources to provide for resident care needs. Provides guidance, counseling and direction, in accordance with organization and department goals/objectives.

Monitor staff’s assessment of the residents’ level of pain and ensures that the pain management regime is effective and timely. Monitors pain documentation for accuracy.

Evaluates performance of staff on a going basis and completes employee’s appraisal in a timely manner according to hospital policy and procedures. Conducts conferences to develop plan of care and to promote staff development.

Collaborates with other members of the health team to develop plans, achieve patient outcome and to maintain continuum of care.

Demonstrate customer service awareness by communicating and relating to residents, families, visitors, staff and health team members in a polite, caring sensitive and supportive manner.

Maintains professional competence through participation in continuing education and
-
other appropriate learning experiences. Attends all mandatory classes and participates in the orientation/training of staff. Seek out higher education.

Demonstrates safety awareness and participates in implementing facility and department safety programs to improve patient safety and reduce errors.Identifies actual or potential environmental hazards such as but not limited to broken medical equipment, wet floors, frayed electrical wires, cluttered hallways, improper plugging of ventilators into outlets, and takes appropriate actions.

Monitors people traffic on the unit to ensure only authorized individuals are present (ID bands, employee ID badges, visitor passes).

Identifies and monitors those patients at risk for wandering, falls, and pressure ulcers.

Implements measures to minimize the potential for medical errors:
-
Safe administration of care through proper identification of resident including allergy status.

Ensure patient/resident is wearing ID band according to policy
-
Educate patient/family about their role in improving safety and involve them in the care.
-
Report errors in a timely manner.
-
Adheres to Infection Control, safety protocols for fire and disaster.
-
Provides privacy to patients at all times and maintain the security and confidentiality of resident information.

Participates in performance improvement activities and other committees.
-
Performs other professional nursing activities as delegated.
Minimum Qualifications:
-
A valid New York State license and current registration to practice as a Registered Professional Nurse in New York State; and
-
a. A Bachelor’s of Science degree in Nursing or higher (or other nursing degree, as identified by NYS Education Department including Master of Science in Nursing, Doctor of Nursing Practice, Doctor of Philosophy Degree in Nursing or Doctor of Nursing Science) from an accredited college or university, registered with the NYSED; or

b. Obtains a Bachelor’s of Science Degree in Nursing or higher, as described in “a” above, within ten (10) years of the date of initial licensure; and

Holds, or obtains through facility orientation, a valid and current certification in Basic Life Support

(BLS) through the American Heart Association (AHA).
