Essential Duties and Responsibilities:
· Participates in daily patient care activities, including the management of anticoagulation patients, as well as other disease states. Identify, prevent, assess, and resolve medication-related problems to ensure a safe and optimal patient outcome in a variety of ambulatory care clinics. Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication and drug interactions. Discuss medication management strategies with the prescriber, documents all changes in patient records.
This pharmacist will monitor medication effects, interview patients, perform basic physical assessments, make dosing adjustments under an approved scope of practice, order laboratory tests, confer with the medical team members as needed, document clinic visits to include the assessment and plan, and arrange follow-up appointments.
Provide health care, or medication-related, education for the patients, or other health care professionals, as needed.
Document clinical pharmacy services and therapeutic consults in electronic medical record, as appropriate
Prevent, detect, monitor, document, and report adverse drug reactions and medication errors.
Participate in the planning and implementation of system-wide pharmaceutical care programs to optimize drug therapy outcomes.
Maintain and update patient profiles with demographics, allergies, and current medications, as needed.
Responds to medication related inquiries from other health care professionals and patients. Document clinical interventions and cost avoidance as part of the pharmacy department documentation program.
Counsel patients on medication, as necessary
Reviews prescriptions issued by Physician, or other authorized prescriber to assure accuracy and determine formulas and ingredients needed.
Demonstrates knowledge and skills to provide developmentally appropriate assessment, care, and treatment as defined by the department to neonates, pediatric, adolescent, adult, and geriatric patients.
Compounds medications using standard formulas and processes such as weighing, measuring, and mixing ingredients.
Directs pharmacy workers engaged in mixing, packaging, and labeling pharmaceuticals.
Answers questions and provides information to pharmacy customers on drug interactions, side effects, dosage, and storage of pharmaceuticals.
Maintains established procedures concerning quality assurance, security of controlled substances, and disposal of hazardous waste drugs.
Enters data such as patient name, prescribed medication, and cost to maintain pharmacy files, charge system, and inventory.
Instructs medical personnel on matters pertaining to pharmacy.
Aids in the implementation of hospital and pharmacy policies and procedures and Therapeutics Committee.
Prepares IV solutions.
Participates in the Performance Improvement Process.
Works well with others in a team effort.
Demonstrates awareness of and compliance with hospital policies and procedures, assumes responsibility for learning new policies and procedures.

Education and/or Experience:
Graduate of an accredited college of pharmacy.
PharmD or equivalent combination of education and experience preferred.
PGY2 in ambulatory care, board certification preferred OR 3 years in ambulatory care experience.
Experience using computer applications including word processing, spreadsheets, and pharmacy information systems required.
