We are currently seeking a highly qualified and compassionate Social Worker to join our well-respected Palliative Care LINK Team, and also assist in other offices as needed.

Job Overview*
Title: LINK/Float Social Work
Location: Waterloo, Iowa
(may be assigned to help at any CVH location as needed)
Job Schedule:
- Full Time (1.0 FTE)
- Monday – Friday, 8:00am – 4:30pm
- Cedar Valley Hospice offers flexible work arrangements to work around the needs of your family! Four 10-hour days and adjusted work hours are available.
- This position will also be part of an on-call rotation for either weeknights or weekends and also for holidays.

Responsibilities*
- Provides and documents initial and on-going assessment of the psycho-social status of the patient and family.
- Provides case management to assist patient and family in accessing community resources
- Develops and implements goal directed interventions based on assessment findings and patient/family needs/goals.
- Supports the educational and outreach goals of the palliative care program by acting as a spokesperson for the program.
Qualifications
Required
- Bachelor’s degree in Social Work from an accredited program (BSW) with one year experience in a health care setting.
- Positive attitude and team player.
- Demonstrate good written and verbal communication skills.
- Basic computer skills with the ability to learn work-related software and have basic typing skills.
- Current Iowa driver’s license, reliable vehicle, and car insurance.
Preferred
- Master’s degree in Social Work from an accredited program (MSW).
- 2 years of recent experience as a Social Worker in a related field.
- LMSW/LBSW licensure
- Experience working in a team environment.

Physical/Other Requirements*
- Ability to move 25lbs with or without accommodation.
- In an 8-hour day, employee may remain in a stationary position 1-5 hours and move about inside CVH site offices/locations, meeting spaces, patient residences, and healthcare facility locations 3-7 hours (or vice versa).
