Unit Overview:
The General Medicine Geriatric unit is a dynamic 22-bed medicine unit with telemetry dedicated to patients aged 65 years and older. Our unit embodies a culture of teamwork, with a core commitment to delivering compassionate care to our esteemed patient population. Our mission revolves around preventing functional and cognitive decline while effectively managing acute illnesses. We achieve this mission by prioritizing the patients' 4Ms – Mobility, Mentation, Medication, and What Matters Most.
Role Summary:
As a Registered Nurse on the General Medicine Geriatric unit, you will play a pivotal role in providing specialized nursing care to patients with acute medical issues. Your focus will be on implementing geriatric nursing interventions aimed at enhancing the well-being and quality of life of our older adult patients.
Key Responsibilities:
- Conduct comprehensive assessments of geriatric patients, considering their unique needs and vulnerabilities.
- Develop and implement individualized care plans tailored to promote optimal health outcomes and prevent decline.
- Collaborate closely with an interdisciplinary team, including the Yale School of Medicine Geriatric Teaching Service, to ensure holistic and coordinated care.
- Utilize evidence-based practices to address the 4Ms framework (Mobility, Mentation, Medication, and What Matters Most) in patient care delivery.
- Actively participate in educational initiatives and resources aimed at enhancing geriatric nursing competency and promoting certification opportunities.
- Foster a supportive and nurturing environment for patients, families, and fellow team members, fostering a culture of empathy, respect, and collaboration.
- Demonstrate a commitment to professional growth and development, seizing opportunities for advancement within the unit and broader healthcare landscape.
Qualifications:
- Valid RN license in the state of Connecticut
- Graduation from an accredited School of Nursing
- All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire.
- Strong interpersonal and communication skills.
- Ability to work effectively in a fast-paced and collaborative environment.
