Long Term Care Pharmacist

TrueMed is seeking a motivated, team oriented, part time Pharmacist to join their winning team in Urbandale, IA. A long-term care pharmacist is responsible in providing day-today operational needs with the dispending of prescription medication as required in meeting the needs and requirements of a long term care pharmacy, adheres to the company’s qualities and customer service standards, policies and procedures, state and federal guidelines.
Essential Job Functions
· Be the drug information expert in the pharmacy: answer staff and facility questions, make sure orders are clear and clinically appropriate, release entered orders from work flow doing all necessary interaction, allergy, and duplicate therapy monitoring and ensure all orders are entered correctly.
· Responsible that all pharmacy laws are being followed at all times.
· Check all packaged medications for proper medication, strength, and lot and expiration date information for accuracy before finalizing for delivery.
· Perform clinical calculations as required- creatinine clearance dosing recommendations, IV dosing and monitoring, warfarin dosing, and other specialty requests.
· Monitor and help implement formulary interchanges.
· Promote and maintain professional attitude and performance from all staff members.
· Perform Medication Regimen Reviews for patient acute change or fall requests.
· Be responsible that the deliveries are not sent until all new orders have been processed and meds are being sent.
· Be in the on-call rotation for after-hours coverage
· Perform Centers of Medicare and Medicaid Services (CMS) required Medication regimen as required by law.
· Ability to work cooperatively as a member of a team.
Non–essential Job Functions
· Ability to work cooperatively with pharmacy and store staff
Requirements
· Degree in Pharmacy or Doctor of Pharmacy degree from an accredited college
· Valid pharmacist license which must be in good standing with the state pharmacy board
· Must be detail oriented
· Must be able to communicate well with patients and staff
