Job Description

Tracks/trends referrals/admits/discharges
-
Coordinates and collaborates with referring Case Managers and Discharge Planners. Cultivates and maintains relationships with referral sources

Conducts outreach with hospitals and referral sources on a routine basis/as able
-
Reviews/assesses clinical records/discharge summary before admission to assess appropriateness for placement

Partners with DON/ADON/MDS Coordinator to review appropriateness for placement

Fully completes admission orders for new admit/readmits.

Obtains insurance authorizations for new admission(s)

Identifies opportunities for new partnerships

Coordinates and oversees Infection Prevention program and Antibiotic Stewardship and reports to DON.

Monitors and provides support and guidance to nursing management to ensure nursing staff are compliant with their respective job descriptions, and working within policy guidelines and performance standards.

Facilitates in nursing department's Quality Assurance program which includes providing support and guidance to nursing staff for conducting audits, identifying problems, and developing plans of corrective action.

Ensures that licensed staff complies written procedures for the administration, storage, and documentation of controlled drugs.

Assists with the Assisted Living Admission Process, as required.

Completes Admission Agreements for SNF and AL admissions.
Qualifications
-
Current RN License from the State of MD.

Current CPR Certification, First Aid Certification

Infection Prevention training or within 30 days.

Computer Literacy
