The Patient Care Coordinator functions as an integral role in facilitating provision of quality patient care by coordination of all patient services. The following serves as a representative summary of the major duties and responsibilities of the Patient Care Coordinator position, however individuals may be required to perform additional duties as requested not included in this general description.
ESSENTIAL JOB FUNCTIONS:
1. Directs all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
2. Receives case referrals via phone, fax, email, and online referral portals. Reviews available patient information related to case, including disciplines required, to determine homecare needs.
3. Data entry of referral information into electronic medical record system, including patient demographics, diagnosis, medications, and specific orders related to patient care.
4. Coordinates to schedule start of care, resumption of care, and multi-disciplinary evaluation visits
5. Coordination of daily schedules by working closely with team of field staff.
6. Manages on-call schedule for field staff and ensures adequate coverage of on-call responsibilities
7. Manages tracking of lab results and reports the required results to designated parties
8. Ensures provision of quality patient care through coordination of scheduling tasks such as Recertification visits, Timed Visits (visits which clinicians must be present at certain times, Visits with specific requirements (such as labs, IV, etc.)
9. Obtains and writes physicians orders, including appropriate scheduling of the requested task (such as lab draw, visit to perform wound care, etc.)
10. Establishes and maintains positive working relationships with current and potential referral sources
11. Acts as a back-up to Patient Care Coordinators and/or Receptionist in their absence
Position Qualifications:
1. At least one year experience in health care office setting, preferably in home
care operations
2. Ability to market aggressively and deal tactfully with customers and the community
3. Knowledge of governmental regulations, CHAP standards, and private payer practices
4. Demonstrates good communication, negotiation, and public relations skills
5. Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities
(Associates Degree, Medical Assistants, Health service Management, Associates in public health)
ONCE A MONTH YOU WILL BE REQUIRED TO TRAVEL TO OUR QUAKERTOWN OFFICE FOR THE STAFF MEETING.
