Northbound is looking for a Primary Therapist. The Primary Therapist provides assessment, crisis intervention, addiction education and group facilitation, family contact and engagement, referent contact and engagement, individual counseling, discharge planning services
DUTIES & ESSENTIAL JOB FUNCTIONS:
· Group Facilitation: Facilitates didactic and counseling groups, and leads family program sessions and group as assigned.
· Individualized Treatment Service Delivery: Meets with clients on assigned caseload on a weekly basis to provide individual counseling. Oversees all aspects of care for clients on assigned caseload, including assessments, individual counseling (1-2 sessions weekly), crisis intervention, continuing care planning and all required documentation.
· Family Interaction/Support: Contacts family with clinical updates (with proper consent) and leads Family Program Sessions as assigned. Provides clinically informed communication to enhance family’s understanding and support of addiction and dual diagnosis treatment, progress, and continuing care recommendations. Provides referrals for families as needed to community support services and resources.
· Coordination: Participates in staff conferences to select, implement, and evaluate treatment planning and progress. Works across departments where needed to ensure a smooth course of care for clients. Provides clinical updates to case manager on agreed upon cadence.
· Individual Case Management: Provides individual case management services including:
o Discharge and continuing care planning
o Coordination with internal departments, external providers, and the brokering of needed resources to meet clients’ individualized needs
o Client deviations, passes, and MOB requests (including verification of MOB requests with appropriate parties)
o Collaborates with and provides updates to referents (with proper consent) at designated intervals
o Communication with external entities including but not limited to employers, probation officers, attorneys, and sober living homes (with proper consent)
· Compliance: Charts all treatments and progress in accordance with Joint Commission, state, and internal regulations and procedures.
· Record Keeping: Prepares reports, treatment plans, case summaries and maintains ongoing confidential records.
· Schedule: Requires after hours and weekend work and assigned holidays as needed for adequate client care
· Treatment Model: Must have understanding of the medical disease model, dual diagnosis treatment, and the 12 step recovery process
· Other duties as assigned.
MINIMUM QUALIFICATIONS:
Minimum of two years experience in the use of psychosocial assessment and psychotherapeutic methods and measures in a hospital, clinic or agency. Dual Diagnosis experience and 12-step knowledge is required. Significant experience with chronic disorders preferred. Previous clinical responsibility to include crisis intervention, individual and group psychotherapy. Excellent interpersonal and communication skills required as well as knowledge of social service agencies, state regulations, professional board standards as is related to client treatment, patient rights, and client/patient confidentiality.
REQUIRED LICENSES/CERTIFICATIONS:
o CPR and First Aid
o Masters in Social Work or Marriage and Family Therapy, Counseling, Counseling Psychology, Psychology, or Addiction Studies required from an accredited college or university
o Valid Clinical Social Work, Marriage and Family Therapist license/Registered MFT Associate Number
