We are looking for an experienced Nurse Practitioner or Physician's Assistant who wants to join an established team. Responsibilities
Our Nurse Practitioner (NP) is in an integral position within the company. The NP confers with physicians, and patients in person and by phone and email, and collaborates with others in the clinic to assure the smooth flow of patient care consistent with established quality indicators. The NP provides oversight as well as the direct coordination of patient care. Working at the clinic will give you the opportunity to deliver higher-value care by spending more time your patients. Essential Responsibilities
- Contact patients by telephone to determine the need for treatment.
- Responsible for the evaluation, diagnosis, prescribing, treatment, and follow-up of patients.
- Maintains ongoing patient follow-up of the patient's clinical information.
- Performs assessment/data collection in an ongoing systematic manner, focusing on the holistic assessment including physiologic, cultural, spiritual, and cognitive status.
- Consults with referring physicians and other clinicians to discuss clinical protocols, patient needs, assessment findings, and to obtain treatment notes.
- Oversees the treatment of patients and provides education to the staff, including appropriate prescriptive practice to achieve optimal clinical outcomes for the patient.
- Provides appropriate and timely documentation of the treatment plan and patient progress.
- Manages the practice management software to assist with patients and provide new patients’ intake process.
- Ensures that records are detailed and complete, complying with related standards and regulations.
- Initiates as well as collaboratively shapes the patient and staff experience to maximize both individual and clinic-wide improvements as needed to achieve optimal healing for the patients.
- Builds ongoing relationships and provides education to the healthcare team members, local physicians, and medical and community organizations.
- Demonstrates proficiency in the use of computers and telecommunication modalities in documenting, tracking, and conveying information.
- Performs other related duties as necessary.
- Advises patients on recommendations to prevent illness and maintain good health.
Job Type: Full-time
Pay: $72.00 - $95.00 per hour
Expected hours: 16 – 40 per week
