Job Summary: The Client Care Coordinator is responsible for scheduling and coordinating caregivers, handling client inquiries, conducting non-medical in-home assessments, introducing care staff to clients, and resolving any issues that may arise between clients and their caregivers. Key Responsibilities:
- Manage the referral and intake process for clients requiring home care services.
- Supervise non-medical direct care personnel and contractors, ensuring adherence to the Non-Medical In-Home Plan of Care.
- Schedule client cases with appropriate field employees, ensuring an efficient and smooth schedule.
- Establish and monitor personnel responsibilities and performance against the Plan of Care.
- Promote effective Home Care services in compliance with agency policies and California State licensing laws and regulations.
- Utilize community resources for continuity of client care.
- Participate in quality improvement activities and agency evaluations.
- Investigate and address problems, policy violations, and reportable events with appropriate corrective actions.
- Participate in after-hours and weekend on-call schedules.
- Facilitate inter-agency and intra-agency communication for appropriate client care.
- Attend office meetings and assist with the application and hiring process.
- Obtain client satisfaction surveys and maintain client charts to ensure compliance.
- Review daily time cards of caregivers and finalize payroll schedules.
- Resolve client complaints/concerns and document actions clearly.
- Perform other administrative tasks as assigned.

Qualifications:
- Minimum of two years of experience supervising Home Care Aides/Caregivers.
- Ability to perform non-medical client assessments and document health, social, and environmental needs.
- Experience in staffing and managing multiple projects using scheduling software.
- Proficiency in Microsoft Office Suite and computer software systems.
- Self-motivated with strong organizational skills, flexibility, and the ability to manage stress and changing priorities.
- Knowledge of California State Home Care Licensure regulations.
- Experience in managing and training staff.
- Proven ability to handle multiple projects and meet deadlines.
- Strong judgment and decision-making skills.
- Commitment to excellence and high standards.
- Excellent written and verbal communication skills.
- Ability to communicate effectively with clients, families, caregivers, and administrative staff.
- Willingness to participate in on-call rotations, including weekends, nights, and holidays.
- Participation in client case conferences as requested.
- Ability to pass a background check through the State of California and maintain a good driving record.
