Job Summary:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary services and resources to improve their quality of life. This RN will admit, recertify, perform Resumption of Care visits as well as discharges and record visits on Kinnser, we will be glad to help you learn the program if you need it.
Responsibilities:
- Conduct assessments to determine clients' needs and develop individualized care plans

- Provide crisis intervention and support in challenging situations
- Utilize motivational interviewing techniques to empower clients to make positive changes
- Collaborate with community resources and agencies to enhance client outcomes
- Maintain accurate and confidential client records
Requirements:
RN with active license
- Currently car insurance,
CPR, Driver's license.
- Knowledge of Medicare
- Strong communication and interpersonal skills
- Ability to handle crisis situations with empathy and professionalism
- Familiarity with motivational interviewing and crisis intervention techniques
