Aging & Adult Care of Central Washington is hiring a Community Services Specialist to join our team in the East Wenatchee office. Our Community Services Specialists provide case management services to clients in the following programs: Family Caregiver Support, Medicaid Alternative Care, Tailored Supports for Older Adults, and other Senior Information and Assistance services.
Responsibilities include:
- Conducts person-centered screenings and assessments to inform callers and clients of all available options, including Agency services and programs.
- Supports callers and clients’ independent decision-making of the best services and programs to meet needs and preferences.
- Screens and assessments may be conducted by phone, in-office, or during home visits.
- Community outreach & events, and providing information and referrals to community resources.
Education and Other Requirements:
- A Bachelor's Degree in Social Work, Social Science or related field and two years of paid social service experience (please see job description for more details).
- Requires a valid WA State Drivers License and auto liability insurance.
- Requires a personal, reliable vehicle.
- Ability to pass criminal background check.
- Bilingual English/Spanish preferred.
