Overview:
We are seeking a highly skilled and compassionate Fulltime Hospice Registered Nurse Case Manager to join our healthcare team. As a Hospice Registered Nurse Case Manager, you will be responsible for coordinating and providing comprehensive care to patients, ensuring their needs are met throughout their healthcare journey. Duties:
- Conduct patient assessments to determine their healthcare needs and develop individualized care plans
- Monitor and evaluate patient progress, adjusting care plans as necessary
- Collaborate with healthcare professionals, including physicians, specialists, and social workers, to ensure coordinated and effective care
- Provide education and support to patients and their families regarding their medical conditions and treatment options
- Coordinate referrals to other healthcare services or community resources as needed
- Advocate for patients' rights and ensure they receive appropriate care in accordance with established standards and regulations
- Document patient interactions, interventions, and outcomes accurately and timely
Experience:
- Minimum of 2 years of nursing experience in a clinical setting, preferably in areas such as ER, ICU, or acute care
- Strong knowledge of physiology, patient assessment, vital signs monitoring, and patient care techniques
- Experience working with managed care organizations is preferred
- Excellent communication and interpersonal skills to effectively collaborate with healthcare professionals and patients
- Ability to prioritize tasks, manage time efficiently, and work independently
- Proficient in using electronic medical record systems
