Job Summary
Works to raise awareness of the Early Success Coalition Network programs and Doula Support Network providers and connect community members with the services provided by member organizations. Supports and implements the activities of the ESC Network (ESCN) and Doula Support Network (DSN) by establishing and maintaining relationships with community organizations, supporting quality improvement activities, facilitating access to services for pregnant women and families with young children, serving as a liaison between community connectors, and assisting with collecting and monitoring ESCN data.
Qualifications
Bachelor’s degree in Social Work, Child Development, Psychology, Counseling, Sociology, Public Health, or a closely related field.
Two (2) years of relevant job experience is preferred.
Supervisory Responsibilities
This position does not have supervisory responsibilities.
Essential Job Functions
Serves as a liaison to community agencies for the Early Success Coalition Network (ESCN) and Doula Support Network (DSN).
Facilitates outreach efforts to enroll pregnant mothers and families with children ages 0-5 into community prenatal and early childhood programs/services.
Maintains contact with community agencies and providers in the network.
Trains referring agencies on the referral process and network screening tools.
Monitors the referral process.
Troubleshoot problems with the referral process.
Supports the team and Community Networks Manager by providing assistance to designated representatives at partner agencies.
Develop and implement an outreach plan to support the community agencies/providers and their outreach efforts.
Coordinates implements, and attends community outreach activities.
Recruit community partners for community outreach.
Documents and reports the activities of the ESC Network and Doula Support Network.
Monitors and maintains the collection and reporting of the group’s activities, applicable data, and outcomes.
Assists in the development of community partnerships.
Support outreach efforts by promoting ParentPlus, including but not limited to, developing digital content for ParentPlus platforms, assisting in content creation for the website, presentations, and flyers, and promoting events that support the Early Success Coalition and partners.
Participate in the coordination of events related to Early Success Coalition programs.
Maintain and report on data related to goals for the Early Success Coalition Network and Doula Support Network including number of trainings, pre-and post-test evaluations of trainings, and referral sources, types, and numbers.
Contribute to team effort by performing other duties as assigned.
Knowledge, Skills & Abilities
Basic understanding of maternal health, child development, adolescent development, mental health, and the dynamics of child abuse and neglect, and trauma.
Program development skills and community collaboration experience.
Community outreach experience is strongly preferred.
Knowledge of community resources and how to access them.
Culturally competent to serve diverse populations.
Interpersonal skills to conduct interviews and facilitate groups.
Organizational skills to coordinate timely responses to multiple activities.
Excellent communication skills (oral and written).
Comfortable presenting to and speaking with large groups.
Must be proficient in Google Suite programs including Docs, Sheets, and Slides.
Physical Demands
While performing the duties of this job, the employee is regularly required to sit and talk or hear. The employee frequently is required to use hands to finger, handle, or feel. The employee is occasionally required to stand and walk. The employee must occasionally lift and/or move up to 20 pounds. Specific vision abilities required by this job include close and color vision. Work in a non-smoking environment.
Special Conditions
Maintain a valid driver’s license with a good driving record with appropriate level of insurance.
