About the Role:
The MIC Community Care Manager is a clinical operations leader. You will serve as the business leader for your state, directing team operations and leading a multi-disciplinary team of clinicians. You will play an integral role in operationalizing MIC’s services, aligning goals and impact with markets as a key stakeholder, and ensuring our care teams are supported in their community based work. You will be an expert shift supervisor who oversees and executes our field-based team’s operational and administrative needs, and will also do so much more. You will help to elevate MIC team operations by constantly looking for opportunities to improve our processes and helping to devise and implement the solutions. The key function of your job is to ensure our clinical delivery model runs smoothly and support the team that provides direct services to Cityblock’s members through our innovative care model.

Serves as the manager and leader for a group of highly skilled, field-based Community Care Clinicians (EMTs, Paramedics, RNs, MAs)

Onboards new team members, and assists with clinical mentoring for existing staff

Serves as an operational leader within the MIC team; identifies problems to solve and helps devise and implement solutions

Brings curiosity to problem solving by identifying key information required, tracking down the information, investigating the information, and synthesizing findings

Proposes solutions that will fix root problems

Collaborates with Central Ops, Market Leads, and market-based teams to solidify and standardize operations across markets

Ensures all processes, workflows, and initiatives have clear documentation, approved by manager, to accompany their launch

Successfully launches new workflows by training the team and relevant stakeholders, as well as defining clear metrics to track the workflow’s success

Enables the team to deliver on their day to day operations, with the goal of decreasing unnecessary hospitalizations, emergency room visits, and speciality referrals. Team scope:

Deploy to members’ homes in response to requests for medical, behavioral health, and social needs. Also assists as needed with scheduled primary care visits, such as clinical intakes.

Provide high-quality, person-centered, integrated physical, behavioral and social care to high-risk individuals in their homes

Collaborate with an interdisciplinary community-based team under the direction of experienced physicians to rapidly respond to the acute and acute-on-chronic needs of Cityblock members

Utilize Cityblock’s custom-built care facilitation platform, which will enable you to collect data, organize information, track tasks, and communicate with staff, patients, and family

Holds responsibility for managing our relationship and utilization with the local market team

Helps craft plans to improve performance

Manages progress against shared plans; identifies roadblocks and leads shared problem solving

Builds intimate knowledge of market operations, and uses that knowledge to identify opportunities for deeper alignment and integration with MIC

Professionally navigates difficult conversations with the market

Reliably completes all administrative tasks on time, without oversight. Administrative tasks include:

Schedules clinical team, and optimizes schedule to match demand and achieve full Unit Hour Utilization
-
Handles ordering and management of medication, equipment and inventory
-
Manages physical space(s)

Manages fleet vehicles
Requirements for the Role:
-
Paramedic, NREMT certified and/or with an active license to practice in the state in which you are seeking employment with Cityblock
-
5+ years of field clinical experience as a paramedic
-
Strong critical thinking skills, excellent communication skills, and strong management of complex clinical issues, providing effective handoffs and ensuring loop-closure

Experience and credibility to identify holistic opportunities to deliver clinical and social impact for each member you interact with

Ability to adapt and operate in a fast-paced, evolving work environment
-
A reputation for delivering excellent clinical service with follow-through and diligence
-
Strong, proven analytical skills, and aptitude for learning new technologies

A deep commitment to transforming health and social care for vulnerable populations

Experience in managing or supervising clinical staff/teams

Valid driver's license and good driving record

Be able to work remotely and in physical workspaces

You have experience working in community paramedicine or non-traditional care settings for paramedics

This role is based in Ohio
We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for health insurance, life insurance, retirement benefits, participation in the company’s equity program, paid time off, including vacation and sick leave. The actual offer will be at the company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location. The expected salary range for this position is:
$94,518 - $118,000
