Title: RN- Home Health- Nurse Assessor
Duration: 5 Years
Training: One Month (3 Weeks Remote & 1 Week Shadow)
Shift: 8:30 am - 5:00 pm Hybrid (3 days onsite and 2 days Remote)
Location: Queens, NY (Hybrid)
Reports To: Regional Nurse Manager – Nurse Ops Manager
JOB SUMMARY:
The Nurse Assessor is responsible for conducting accurate assessments for consumers seeking community based long term care services and supports.
JOB RESPONSIBILITIES
1. Perform initial assessments of consumers that includes evaluation of health status, functional status, strengths, care needs, and preferences.
2. Review consumer medical documentation and/or health referral forms, as relevant to the case.
3. Enter assessment data into the UAS-NY online application and transmit, as required.
4. Code the UAS assessment correctly based on training and guidelines in the UAS-NY Community Health Assessment Reference Manual.
5. Document outcomes of calls to consumers and scheduled appointments.
6. Maintain current knowledge of regulatory and project changes pertaining to quality and validity of assessments.
7. Enhance communication and collaborative relationships with leadership and interdisciplinary care team members to facilitate receipt of services.
8. Review non-qualified assessments with the Clinical Quality Assurance Department.
9. Maintain a comprehensive working knowledge of community resources and network services for the target population.
10. Perform other duties as may be assigned by the Regional Nurse Manager or other project management
REQUIRED SKILLS:
- Ability to conduct in-home health assessments throughout the assigned region in the state of New York.
- Ability to review health status questions and make evaluations and determinations based on UAS criteria.
- Computer literacy including ability to navigate Web-based applications, Microsoft Outlook & Microsoft Word; ability to navigate and conduct assessments via Zoom.
- Knowledge of the theory, principles, and practices of general professional nursing; knowledge of state and agency laws and regulations governing general professional nursing practices
- Excellent organizational, interpersonal, written, and verbal communication skills; ability to communicate and work effectively with multi-generational consumers.
- Ability to demonstrate sensitivity to the concerns of others.
- Ability to perform comfortably in a fast-paced, deadline[1]oriented work environment; ability to successfully execute many complex tasks simultaneously; and a strong ability to work independently
REQUIRED EXPERIENCE:
- (2) years of experience in a HOME setting within the last ten (10) years
REQUIRED EDUCATION:
- Graduate of an accredited nursing program
REQUIRED LICENSURE
- Current RN Licensure in New York State
- New York State ID or Driver’s License
