*Responsibilities:*
* Conduct office work and at various field locations using portable communication devices
* Utilize online request forms and reporting templates for efficient communication and data collection
* Create a resource package outlining community benefits available to program participants
* Coordinate case management and housing navigation services for each participant
* Prepare and distribute welcome packets for participants entering program
* Develop housing plans that allow for placement for eligible participants in temporary or permanent accommodation
* Arrange for transportation and storage facilities as needed
* Coordinate access to various community services and resources, facilitating a warm hand off after program completion
* Conduct weekly participant wellness checks on each participant enrolled in the program
* Assess and monitor participant progress while enrolled in program
* Undertake training in Trauma-Informed Care, Mental Health First Aid, Infection Control, common knowledge of communicable diseases, and program policies and procedures
* Conduct comprehensive assessments of participants to identify their needs during program enrollment
* Maintain accurate and up-to-date participant records, documenting assessments, interventions, and progress towards goals
* Perform other tasks and duties as assigned on various community health programs
