A Day in the Life:
“I oversee the day-to-day operations of the Home Recovery Care team. I have an awesome team of registered nurses who monitor our Home Recovery Care patients. My team coordinates all necessary elements required for a patient to be cared for in their own home instead of a hospital. The goal is to deliver quality care and stellar outcomes while keeping the patient out of the hospital.”

Responsibilities:
As the Team Leader, you will be responsible for overseeing a tenured team of acute care nurses and care coordinators as well as operations for the hospital at home program for our patients.
- Partners with leadership on operational, clinical, and quality reports and presents market updates on a regular basis
- Facilitates communication and coordination between all members of the care team
- Coordinates referrals and appropriate resources to assist patient and/or caregiver in continuation of care in the outpatient setting
- Ensures that patients have access to appropriate services to meet their provider directed care plan needs
- Maintains all required documentation on provider and patient interactions including visits, intake, interventions, and patient issues
Schedule: Monday-Friday, 8a-5p - NO nights, oncall or weekend rotations!!)
Hourly Rate: $60-65/hour

Qualifications:
- 2+ years of leadership/management experience Required
- 3+ years of acute nursing experience (Emergency, Medical-Surgical, Intensive Care, etc.) required and home health experience preferred
- Current RN license
- Covid Vaccination REQUIRED*

Pay: $60.00 - $65.00 per hour
