ACQC is seeking a Nutrition Educator for their Food & Nutrition Services (FNS) Program. The FNS Program works with vulnerable or hard to reach clients and aims to empower its clients to take control of their diet and overall health. The role of the Nutrition Educator is to is to partner with clients to offer nutritional education, and to provide administrative support to the FNS program.
Responsibilities for the Nutrition Educator
- Contact clients to schedule appointments for pantry pick up
- Perform Intake Assessments and Reassessments; schedule and track follow up visits
- Work with medical providers to ensure clients are linked to medical care
- Ensure pantry bags meet the nutritional standards of the FNS program
- Assist with planning and facilitating nutritional workshops
- Compile and review all nutrition related educational material
- Work with FNS Nutritionists and Peers to ensure program compliance
- Document and maintain client records regarding attendance, participation, progress, accomplishments, additional needs, referrals, and goals
- Supply and record the distribution of food vouchers; ensure clients are meeting FNS standards regarding food purchases and nutrient requirements
- Maintain vouchers and assist with submission to the Finance department
- Responsible for ordering food from vendors, receiving food, and tracking inventory
- Ability to work professionally and confidentially with all clients
- Other duties as assigned by Program Coordinator, or Director of Case Management
Qualifications for the Nutrition Educator
- Bachelor’s degree or Associates degree with 2 years of related experience
- Knowledge or experience working with individuals with HIV/AIDS and other chronic illnesses
- Understanding of HIPAA and Article 27F regulations regarding confidentiality
- Sensitivity to cultural, ethnic, and gender orientation differences
- Bilingual, English/Spanish preferred
This position is a full-time, non-exempt position, and is based in Jamaica, Queens near the E, F, and J trains, and the LIRR.
