We are searching for a motivated individual to join our team as a foster care/community-based program intake coordinator. In this rewarding career you will be helping to enrich the lives of others by assisting in matching clients with foster families or forever homes.
This position does require travel, but you can submit mileage reimbursement.
What Do I Need?
Education and Experience
Bachelor's degree in social work, psychology, human services or related field
OR
Related experience may be substituted in lieu of a degree

- You will be responsible for timely submission of hours worked following agency policy.

What Will I Do?
- You will serve as the primary point on contact for referrals and inquiries regarding foster care placements.
- You will conduct initial screenings and assessments of children referred to the foster care program to determine suitability for placement.
- You will collaborate with caseworkers, social workers, and other professionals to gather relevant information and assess the needs of children entering foster care.
- You will coordinate placement matches bwtween children and foster families based on compatibility and the child's specific needs.
- You will facilitate the placement process, including coordinating transportation, completing necessary paperwork, and ensuring that all parties are informed and prepared for the placement.
- You will provide ongoing support and resources to foster families during the intake process, addressing any questions or concerns they may have.
- You will maintain accurate records of all intake activities, including assessments, placements, and communications with stakeholders.
- You will participate in team meetings, case conferences, and trainings to enhance knowledge and skills related to foster care intake and placement.
- You may be required to attend initial court hearings.
- You may be required to complete initial home visit, and complete casework documentation related to this task.
Hours of Work
9am-5pm, Monday through Friday
On call hours will be Monday through Sunday 5pm-9am. The ability to take at least two weekly shifts of on-call per year is required.
