Essential Duties and Responsibilities:
As a CTM, you will ensure quality care is being provided to patients by reviewing clinical notes to make sure quality assurance standards are met.
- Responsible for conducting an ongoing, comprehensive, integrated agency assessment of the quality and appropriateness of the care provided, including services provided under arrangement.
- Aggregate and trend data collected to correct identified problems in collaboration with management.
- Monitor all service providers for effective interchange and coordination of care in providing care.
- Demonstrates in-depth knowledge of and ensures compliance with all local, state, and federal laws relating to all the operations of the agency.
- Ensures that the clients' plan of care is executed as written
- Reviews OASIS visits, 485s
- Performs patient chart audits.
- Regularly participates in case conference activities and assures through attendance and recorded reviews that case conferences are conducted and documented in accordance with Agency policy.
- Review all reports of complaints, errors, incidents, and grievances to ensure that they are fully documented and properly handled in the period specified by the Agency's policy.
- Assures the confidentiality of clinical records.
- Participates in the development and implementation of a staff educational program for documentation and Quality Improvement.
- Works with administrative and supervisory personnel regarding home care visits to assure high quality and proper follow-up patient care regarding patient needs.
Qualifications
- Have at least 2 years of home health experience within the last 24 months and have a demonstrated understanding of quality assurance requirements related to home care services
- Communicates clearly and effectively (both oral and written) with patients, clients, Team Members, peers, and Leadership.
- Excellent documentation and organizational skills, including time management, multi-tasking, and prioritizing
- Ability to establish and maintain effective working relationships with all segments of the company
- Exhibit self-direction, independent judgment, discretion, and ability to work with multiple teams
- Must maintain a professional and friendly demeanor
- Kinnser/Wellsky Experience Preferred
- OASIS certified and coding a plus
