DESCRIPTION: As a team lead you have been selected as a role model for your department, we expect you to set the standard for the department and the expectations for quality care. Team leads should be setting the example for all other schedule coordinators by preforming their job duties to the best of their abilities. A team leader is required to help run the Case Management department when the Senior Client Case Manger is out of the office.
POSITION QUALIFICATIONS:
•High school graduate or equivalent
•Knowledge of & willingnessto learn basic office procedures & use of equipment.
•Willing to learn basic skills required for specific duties.
•Strong interpersonal, PC and Communication skills (phone marketing and data entry ability)
•Staffing and customer service experience with clients in the medical and/or home health care field REQUIRED.
•Knowledge of the home care/elder care industry REQUIRED.
•Outstanding organizational skills and the ability to work with frequent interruptions.
DUTIES:
•Send daily Morning or Evening summaries-as requested
•Send weekend summary-as requested
•Over see Schedule coordinators-as requested
•Trainingschedule Coordinators-as requested
•Performs basic office duties
•Schedules clients and employees daily
•Responsible for maintaining contact with Social Workers and other Case Managers
•Maintains office filing systems as requested
•Files all documents appropriately
•Answers telephone and relays messages appropriately as required
•Responsible for copying documents as requested
•General typing and correspondence as requested
•Organizes and adequately stores active and inactive records as required
•Maintains confidentiality regarding all agency records
•Managing coordination of client services and emergency scheduling issues
•Managing client case load while proactively growing it.
•Build lasting relationships with clients, referral sources and community organizations
•Recruit field staff to provide services for clients
•Develop strong, communicative relationships with the team
•Promote exemplary customer service to patient and patient family at all times.
•Ensure effective communication and collaboration with office staff daily in regards to client cases and staffing.
•Maintain scheduling by inputting/updating client and caregiver information via scheduling software system.
•Contact and follow-up with caregivers regarding client assignments.
•Document inquiries for service and relay to appropriate staff in a timely manner
•Document any "client events" and relay to appropriate staff in a timely manner
•Document all calls to clients, clients family, coordination agencies in real time.
•Office Administration-which includes answering phones, copying, filing, mailing, etc
•Assist with organization and preparation of office projects
•Needs to be available for on-call schedule which is rotated among office staff which may include weekends and holidays.

Pay: $17.00 - $22.00 per hour
