Description:
Performs activities which support the Case Management/Utilization Management team function. Promotes optimal health care outcomes in accordance with the policies, procedures, applicable laws and contracts, philosophy, objectives and mission of Community First.
This opportunity is full time days Monday-Friday.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Actively identify alternative practice between physicians, nurses, and other health care individuals.
2. Evaluates and documents the patient’s clinical status and post-hospital needs and plans.
3. Monitors delivery of care and the patient’s response to care during hospitalization.
4. Assesses patient’s physical status to revise plan of care.
5. Discusses treatment plan with patient, family and/or significant others.
6. Reports/communicates changes in patient and plan of care to staff members involved in patients care.
7. Educates the medical staff and other members of the team on proper documentation of services rendered.
8. Screens cases for risk management, infection control, or quality of care issues.
9. Identifies discharge planning needs, anticipated discharge outcomes.
10. Performs admission, concurrent and discharge reviews.
11. Communicates and provides updated clinical and medical information and anticipated discharged date.

Education and/or Experience

LSW required, LCSW preferred

2+ years Clinical Social Work required

Acute care Social Work experience preferred

Bachelor’s degree required

EMR/EPIC experience preferred
