RN /MDS Director
Job Summary: The MDS Director conducts and coordinates the development and completion of the resident assessment process in accordance with the requirements of the state and federal regulations as well as the policies and procedures of Healthwin Specialize Care.
: Must have at least 1 year of MDS experience and maintain a license to practice nursing for the State of Indiana in good standing. Possess knowledge of the RAI process from beginning to end including the care planning process and scheduling requirements for state and federal guidelines.
: Knowledge of basic resident assessment process. Excellent communication skills in dealing with nursing staff, administration, department heads, residents and visitors. Must review and interpret OMPP reports, CMI summaries and resident report roster. Identify significant changes, risks and causal factors. Supply PPS information to biller and other departments. Must possess the ability to share knowledge of the needs of the elderly, disabled and confused residents. Display the ability to identify and promote restorative measures. Demonstrate the ability to acquire, maintain and convey knowledge of pertinent state and federal regulations applicable to the long-term care setting.
Areas of Responsibility:
- Educate, train and orient staff on the Resident Assessment Instrument.
- Ensure timely and accurate completion and submission of the MDS to the QIES ASAP database. Monitor for fatal errors and correct errors as needed.
- Obtain validation of MDS acceptance.
- Retrieve quality indicator reports at least monthly or upon request, analyze reports, take necessary action, compare information with past assessments and report the information to the Quality Improvement Committee.
- Monitor regulation changes in a timely manner.
- Coordinate charting process to ensure accuracy and timely completion.
- Actively participate in the development of a comprehensive, individualized interdisciplinary plan of care for each resident. In addition, daily monitoring through the 24 hour report and physician’s orders to ensure resident’s care plans are revised, updated and current to reflect an interdisciplinary approach to the resident’s changing needs.
- Oversee the timely completion of certification and recertification.
- Have the ability to project an admission RUG score and track RUG categories to ensure accuracy for all residents.
- Know the elements of each RUG group and the significance of the hierarchy.
- Articulate the charting requirements to the manager and floor staff to correspond with the MDS assessment requirements.
- Maintain an RAI calendar on the F drive which is accessible to members of the IDT, which provides clear, concise, up-to-date information on completion deadlines for each phase of the RAI process for each applicable resident
- Access Myers & Stauffer/ANNAC website as needed to stay informed of MDS/RAI updates.
- Check and complete all assessments as needed for the quarterly and annual assessments.
- Monitoring of ancillary department sections of the RAI for timely completion, including notification of the individual departments if deadlines are in danger of not being met by the departments. Daily auditing during the assessment period of MDS staff audits and ADL grids to ensure appropriate coding is being documented and correcting any inappropriate or missing coding.
- Monitor the input of each MDS to ensure accuracy and consistency of responses across the interdisciplinary assessment acting as a facilitator to help resolve disagreements around team members.
- Function as a team member of nursing administration.
- Maintain professional competence.
- Carry out nurse and C.N.A. education as directed by the assistant director of nursing or the director of the nursing department to assist in staff education.
- Adhere to safety and infection control policies and procedures.
- Practice and provide guidance for performance improvement.
- Performs patient interviews and assessments as well as gathers assessment information through documentation review and staff/family interviews to conduct quality assessments.
Interpretation of Responsibilities:
- Function as a Nurse Leader.
- Receive report of residents with acute medical conditions, deteriorating status, infections or other situations that require a change in resident condition.
- Assist D.O.N. or A.D.O.N. with bed management, length of stay and Medicare benefit utilization.
- Monitor written MDS assignments to ensure staff has completed the appropriate documentation.
- Provide consultation to assist in the care of special needs residents (MRDD, MI).
- Monitor documentation of resident care flow records, restraint records, medication and treatment administration sheets and bowel protocols for supportive documentation during your chart reviews and report any lack of pertinent documentation to the D.O.N. or A.D.O.N.
- Schedule and conduct care plan meetings according to the MDS schedule.
- Track Medicare days in coordination with the business office for all residents and update the tracking log weekly.
- Determine qualifications for Medicare coverage after therapy is complete.
- Act as a resource for staff for questions pertaining to the MDS.
- Audit and evaluate restorative programs and care plans.
- Attend committees as directed by the D.O.N. or Chief Clinical Officer.
- Monitor Quality of Resident Care
- Assess resident care in light of diagnosis, care plan goals and current status of behavior.
- Update care plans and daily C.N.A. worksheets as needed.
- Function as a member of Nursing Administration
- Keep the Director of Nursing apprised of staff concerns, resident conditions, or environmental concerns as they occur.
- Know and support facility and nursing policies and procedures.
- Refers to and uses state and federal regulations to ensure compliance of the same on all nursing units
- Participates in the development, review, and revision of policies and procedures via the Director of Nursing.
- Provide suggestions and alternatives to improve operations of all nursing units and provision of resident care.
- Demonstrates professional problem-solving ability with staff, residents and family members.
- Maintain Professional Competence
- Keep abreast of development in the field of geriatrics, gerontologic nursing, long-term care and professional issues related to MDS assessment in the long-term care setting.
- Identify and attend continuing education offerings, which enhance the supervisory role in the long-term care setting.
- Present a positive professional image.
- Assist in identifying areas of concern or areas needing improvement.
- Assist in implementing positive changes within the nursing department to enhance the quality of life for the residents as well as the employee environment.
- Assist in evaluating employees as directed.
- Assist in the ongoing improvement of Healthwin Specialized Care.
- Perform Additional Duties as Required
