Statements of Purpose
The Substance Abuse Case Manager is responsible for assisting and supporting residents through the entire treatment experience from admission to discharge. The Case Manager monitors the course of treatment for each resident in a proactive manner to minimize the barriers that might compromise the resident’s treatment.
The Case Manager is responsible for contributing insightful, practical, meaningful information to the treatment planning process through running groups and individual assessments. The Case Manager orientates each resident to the program rules, expectations, intent, and routine of the daily treatment schedule. The Case Manager also assists the client with creating an individualized treatment plan that will outline the client’s goals and treatment objectives.
This position requires a strong knowledge of appropriate personal and clinical boundaries, a consistently positive attitude, and the ability to deal with difficult, uncertain, and stressful situations in a positive manner.
The Case Manager reports to the Clinical Director.
Major Task, Duties and Responsibilities
● Completion of all duties and/or documentation within the guidelines of company and AOD standards.
● Able to present didactic/psychoeducational groups and workshops.
● Provide an individual session once a week for a minimum of 30 minutes to each client.
● Directs residents in a clear, concise, and informative manner.
● Contributes to the Comprehensive Treatment Plan through participation in treatment planning meetings and process.
● Create and implement a discharge plan for the clients and assist in locating the appropriate community resources.
● Contacts residents’ family members to set appointments or educate the family on the residents’ current status and/or progress.
● Ability to self-motivate and possess strong time-management and organizational skills while managing a high volume of client interactions.
● The Case Manager must demonstrate tact, initiative, resourcefulness, professionalism, enthusiasm, and a non-judgmental attitude.
● Responsible for reading the shift log at the beginning of shift and documenting all necessary information at the end of the shift.
● Documentation of all activities and daily progress in the client’s file.
● Crisis Intervention.
● Assist, teach and/or supervise clients with general program activities.
● Other duties as assigned.
Charting Requirements
● Initial Treatment Plan must be complete and signed within 24 hours of admit. Treatment Plan Updates must be completed every 7 days.
● Biopsychosocial must be complete and signed within 3 days of admit.
● If Case Manager leads groups, notes must be logged same day.
● Referents
○ Must contact referent within 48 hours of admit. Call must be documented in client chart.
○ Must notify referent within 72 hours of client’s treatment plan. Call must be documented.
○ Must contact referent minimum of one time per week via phone to keep family and/or referent updated, including weekly update of client treatment plan. Call must be documented.
Skills, Knowledge and Competencies Required
● Knowledge of drug and alcohol dependency, mental health, family systems and various recovery resources in the community such as Smart Recovery/other resources in the community.
● Ability to identify a crisis.
● Fully understands and maintains policies regarding professional ethics, including appropriate boundaries and patient confidentiality.
● Basic knowledge of referrals, both in and out of 90210 Recovery.
● Ability to communicate clearly, with clients, staff, peers, supervisors and non-90210 Recovery resources.
Minimum Qualifications
Following are the qualifications required to perform the essential functions of this position.
Specific qualifications may vary based on assignment.
_____ Must be at least 18 years of age
_____ Certification or licensure in Drug and Alcohol Counseling preferred.
_____ Minimum of two years’ experience in evaluating and/or treating persons suffering from mental illness and/or substance abuse.
_____ Class “C” Driver’s License (if driving for company), otherwise, will need Photo ID.
_____ CPR /First Aid certification required, obtain upon employment and maintain current thereafter.
_____ Criminal Background Clearance required
_____ Health Screen required
_____ Negative TB Test Results required
_____ Develop computer skills adequate to perform word processing duties upon employment.
