Job Overview:
We are seeking a dedicated Case Manager to join our team. The ideal candidate will be responsible for providing support and guidance to individuals in need, ensuring they receive the necessary resources and assistance to improve their quality of life.
Duties:
Have experience with the following, including but not limited to:
Working with homeless and/or special needs populations. Provision of case management and/or mental health services in stabilization supportive housing, including establishment of a crisis response system. Agreement management and compliance. Budgets and expenditure tracking. Providing administrative oversight to ICMS staff. Developing and implementing QCPs. Working closely with other supportive service agencies, including case management and mental health providers to ensure clients’ needs are being met.
Building collaborative partnerships Be responsible for developing policies and procedures and creating forms that facilitate the effective operation of and delivery of high quality ICMS. Have proficient writing and computer skills and could produce reports on a regular basis and as requested by the County. Coordinate with the County staff and its research contractor on data collection and reporting. Data Entry experience. Monitor client progress and adjust plans as necessary. Maintain accurate and up-to-date client records
Requirements:
- Bachelor's degree in Social Work, Psychology, or related field (preferred, not required).
- Proven experience in behavioral health, motivational interviewing, workforce development, mentoring, or related fields
- Knowledge of Medicare, addiction counseling, childcare, Section 8 housing, and crisis intervention
- Strong communication and interpersonal skills
- Ability to work effectively in a team environment
- Certification in social work or related field is a plus
