About the Role
In keeping with our organization’s goals, the Director of Admissions will manage the nonclinical components of the referral process for multiple buildings.
Job Duties

Enters new and return leads into CRM in PointClickCare.

Verifies benefits through PCC for Medicare/Medicaid and through GCHMO’s ClickUp Portal for Managed Care.

Runs NSOR report on all referrals.

Ensures communication with Resident Services Coordinators to maintain awareness of occupancy levels and bed availability necessary for MOT.

Understands the referral process including policy, procedures, assessments, and communication requirements.

Participates in daily CRM calls.

Updates lead status in CRM throughout the referral process.

Inform RDCTs of referral source issues that arise during the referral process.

Coordinates MOT notification with facility on all incoming admissions

Maintain a thorough knowledge of the facilities services, clinical capabilities, and physician relationships
Required Education, Experience, and Skills
-
2 years’ experience with admissions process at a skilled nursing facility, subacute, rehab, or other health care facility.

Significant experience and strong skills using Point Click Care software.

Significant experience and strong computer literacy skills (Microsoft Outlook, Microsoft Office suite, and other online platforms).

Recent long-term care experience required.

Experience making independent decisions when circumstances warrant such action.

Self-motivated and self-directed: possesses the ability to work independently

Excellent written and verbal communication skills

Pay: $65,000.00 - $75,000.00 per year
