Job Summary
We are seeking a Registered Nurse to join our team. In this role, you will primarily be completing new patient assessments as needed and creating a plan of care for individual patients for non-skilled services. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in providing the best recommendations for patient's ADL CNA'S task list.
Responsibilities
- Coordinate with personal supportive programs to assess, plan, implement, or evaluate patient care plans.
- Document vital signs, potential fall risks, skin integrity notes
- Maintain accurate records at the time of re-assessment periods of patients throughout the year and annual.
- Available on a as needed basis/schedule bases
Qualifications
- Currently licensed Registered Nurse (RN) in state of employment
- Excellent interpersonal and client care skills
