Responsibilities
- Serve as the primary liaison between the patient’s pharmacy and the company’s biomedical (HIV, Hepatitis C and PrEP) care team.
- Acts as the patient’s advocate to ensure that the prescribed drug therapy and pharmacy care plans are followed as prescribed and meet the medical, emotional, social, and psychosocial needs of the patient to achieve health equity outcomes and community impact. Special emphasis will be placed on communities of color and LGBTQIA+ populations to achieve viral suppression.
- Assists persons living with HIV, Viral Hepatitis and HIV negative persons on Pre-exposure prophylaxis (PrEP), by coordinating their medical care, assist with medication adherence technology, co-pay assistance programs, managed care insurance enrollment, Drugs Assistance Programs, Medicare, and other health related resources to ensure successful drug therapy outcomes and demonstrate value to patients, healthcare providers and payers.
- Outreach to patients to assess and address barriers to medication adherence, with assistance from the pharmacist as needed
- Assist in contacting patients to ensure collection of all necessary data, communicating with insurance companies and pharmacies throughout the authorization process, and maintaining records regarding the authorization process.
- Ascertain information from patients about adherence to medications, barriers to adherence, and what can be done to eliminate these barriers.
- Answers and resolves patient inquiries within committed time frames. Understands the physical, psychological, and social challenges of specific disease states and assists the patient with drug therapy and pharmacy care issues.
- Procure accurate medication lists for all patients including medications from all outside prescribers. Communicate these medication lists with pharmacy and medical team. Meet with patients at the time of visits and review all home medication bottles when necessary. Assist with pill box preparation when necessary.
- Work hand and hand with the patient’s infectious disease care team to ensure excellent customer service to patients.
- Support strategies to promote medication adherence (Mobile APP enrollment, Mobile App virtual pill box reminder, record qualitative monthly/quarterly feedback, refill reminder).
- Maintain monthly client load enrolled in database, to assure active enrollment and bi-annual re-certifications are met. Provide administrative support to the agency’s Ryan White Non-Medical Case Manager.
· Serve as a subject matter expert in prescription formulary review and resolution of medication access issues.
* Establishes and maintains strong customer relationships with physician practices, pharmacy techs and case managers to streamline communication to the patient, payers, pharmacist, nurse, lab, and other healthcare related parties.
- Troubleshoots and resolves patient and payer issues in a timely fashion. Works in a team-structured environment to provide support to other members of the healthcare team.
- Embraces a continuous quality improvement customer service approach by proactively identifying areas of improvement and communicating those ideas to the healthcare team.
- Maintains current notes and documentation related to patient's drug therapy, pharmacy care plan and adherence incentives; flags issues that need to be addressed.
- Coordinates responses and resolutions with appropriate internal and external parties.
- Actively participates in discussions with contracted pharmacy techs, pharmacy support manager and pharmacy care coordinators, to review cases and related drug therapy and pharmacy care plan issues. Performs related duties as assigned.

Qualifications:
- Active Pharmacy Technician Certification (CPhT) License with The Illinois Department of Financial and Professional Regulation and listed as non-disciplined.
- Experience administering COVID-19 immunizations
- Ryan White Experience preferred.
- Pursuing advanced education in social sciences.
- 340B Experience preferred
- Bachelors in pharmacology or social science.
