Job Description Summary
The Nurse Case Manager is responsible for managing each episode of care including: admission, planning, assigning, directing and monitoring progress of home health care services, and discharging patient from care. The RN Case Manager is experienced in nursing, with emphasis on community health education. The professional nurse case manager builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
Position Qualifications:
- Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting or home health.
- One (1) year experience in home health case management.
- Current CALIFORNIA STATE RN license, CPR certification and valid CA driver’s license.
- Associate or higher degree, with one (1) year of home health care experience preferred.
- Management experience not required.
- Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
- Visual acuity and hearing to perform required nursing skills.
Job Type: Full-time
Pay: From $120,000.00 per year
