Position Overview: The Personal Care Assistant plays a vital role in providing various
services to patients to meet their personal needs, promote comfort, and maintain a safe
environment. This position reports to the RN Case Manager and Client Service
Representatives, working collaboratively with LVNs, RNs, CSRs, Executive Director,
CEO, and Social Workers.
1. Assist with personal care, such as bathing, oral care, eating, and any toileting needs.
2. Assist the patient with getting dressed and undressed.
3. Plan and prepare meals.
4. Assist in feeding the patient.
5. Assist the patient with ambulation and exercise according to their Care Plan.
6. Assist with getting the patient in and out of beds, chairs, and wheelchairs.
7. Assist the client with getting around their home safely, inside, and outside.
8. Provide companionship.
9. Encourage the patient to participate in hobbies or other leisure activities.
10. Assist the client with running errands, doctor appointments, grocery shopping, and more.
11. Assist the patient in the self-administration of medication.
12. Assist with laundry, washing/drying, and folding/putting away clothes.
13. Follow safety protocols regarding the transferring of a client and safely use necessary medical equipment.
14. Report any change of condition to the office during business hours and the on-call phone during after-hours.
15. Complete a care note (ADL) before the end of each shift.
16. Document in the communication binder information regarding the patient's care needs before the end of each shift.
17. Adhere to the Organization’s documentation and care procedures.
18. Adhere to standards of personal and professional conduct.
19. Read and follow written instructions and document the care given.
20. Demonstrate tact, patience, and good personal hygiene.
21. Review the Care Plan at the beginning and throughout the shift to ensure all tasks are completed.
22. Provide thorough verbal shift change reports to care staff coming on shift.
23. Any added assigned tasks by the RN Case Manager and/or a Client Service Representative (CSR).
Standards and Accountabilities:
1. ADL’s will be performed daily.
2. Documentation will be completed daily before the end of each shift and electronically turned in. All paper forms must be turned in before noon on Mondays.
3. Changes in client conditions will be immediately reported to the CSRs and/or the on-call staff according to the proper procedure of the agency.
4. The client binder, including the care plan and communication logs, will be reviewed at the beginning and end of a shift.
5. Logging in and out through the agency telephony system will be done at the beginning and end of every shift using the Generations app. The client’s phone can be used if there are technical issues with the app. The app must be downloaded to your phone.
6. Driver license, vehicle insurance, and certification updates will be provided to Human Resources.
7. All mandatory training, online or in-person, will be completed within the assigned timeframe.
8. Effective teamwork between caregiving, CSRs, administrative staff, and nursing staff will always be fostered
JOB LIMITATIONS:
The Personal Care Assistant will not function in any manner viewed as the practice of nursing according to the State’s Nurse Practice Act. Specifically, the Personal Care Assistant will not administer medications, take physician’s orders, or perform procedures requiring the training, knowledge, and skill of a nurse, such as sterile techniques, or end-of-life medications.
Educational Requirement:
A high school graduate or equivalent.
