Essential Job Functions:
- In coordination with other teams and in compliance with court orders and governing regulations, develop ISPs for review and approval.
- Function as a liaison between all parties associated with the youth’s participation in the program including parents, DHS staff, managed care, probation officers, court management, etc.
- Monitor youth throughout their daily activities and take action to recognize positive progress or sanction negative behaviors.
- Discuss and provide counsel to youth regarding the cause and effect of various behaviors, personal and family values, attitudes, and lifestyle.
- Facilitate connections to services that will support progress toward achieving goals.
- Keep confidential files of all required documentation, including records of activities and behavior, incident reports, progress reports, summaries of monthly and quarterly reviews, etc.
- Attend court hearings, DHS meetings, team meetings, etc.
- Coordinate discharge plans to promote successful transition.
Role Requirements:
- Bachelor’s degree in a related field such as social work, human services, etc.
- 1+ years of case management experience
- Valid driver’s license, daily access to a reliable vehicle, and car insurance
- Bilingual in Spanish/English is a plus
Salary: $45,000.00 per year
