The CCBHC Community Partner Referral Specialist will be responsible for overall community partner outreach for Metrocare. The Referral Specialist will work with the Community Partner Manager, RN Care Coordination Manager, and other staff to develop a vision for community engagement.
To ensure a seamless transition to Metrocare occurs, the Care Coordination Team will include a Community Partner Referral Specialist, solely responsible for ensuring coordination of referrals received from community partners, ensuring transition for individuals coming into services utilizing an organized, person-centered approach that promotes engagement, support, and understanding.
The Community Partner Referral Specialist is a member of the Care Coordination Team. They are responsible for working with adults and families in treatment in a manner that is trauma-informed and responsive to needs, culture, gender, and military status. The Community Partner Referral Specialist position is responsible for applying a holistic approach in service delivery.
The Community Partner Referral Specialist will receive referrals from external agencies, document information regarding referral and schedule initial appointments, and coordinate referrals with Community Partner Manager, RN Manager and Center’s Clinical Managers to ensure continuity of care.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

Establish and continue to build partnerships and meaningful relationships with related organizations throughout Dallas County and the state by co-locating at partner sites.

Initiate, support, and/or collaborate with staff, volunteers, and outside partners on emergent community.
- Support Community Partner Manager, RN Care Coordination Manager, Clinical Managers and Transition Liaison Manager with duties and assignments as assigned.
- Receive community referrals, review for complexity and assign to the appropriate staff.
- Interface routinely with the Community Partner Manager, RN Care Coordination Manager, CMIIIs, and other site managers to ensure the program operates at maximum capacity and efficiency.
- Routinely utilizes advanced communication skills both written and verbal, and problem solves daily.
- Coordinates with community partner, setting up, and ensuring a consumer receives his/her psychiatric assessment, intake and/or follow up appointment(s). Coordination attempts must include effort to schedule a follow up appointment with a Metrocare provider within 7 business days of the individual’s referral.
- Assistance with scheduling and completing all internal and external provider appointments.
- Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services.

Coordination with internal and external providers to ensure a holistic approach to care that considers all areas of an individual’s life, the individual’s wants and goals and all treatment interventions.
- Documenting services in the electronic health record within 24 hours after services have occurred.

Helping to maintain a list of community and agency resources for successful referrals.

Maintains confidentiality of information concerning consumers and family members

Conduct job responsibilities in accordance with the ethical standards of conduct, state contract, appropriate professional standards and applicable state/federal laws

Keeps the Community Partner Manager aware of all issues that confront any team and/or program.

QUALIFICATIONS
EDUCATION AND EXPERIENCE:
- High School graduate with some college or technical school/training with a minimum of 1 year of progressive customer service experience required. Experience in an office or MH/MR setting preferred.

Bilingual English and Spanish preferred

Bachelor’s Degree preferred.

CERTIFICATIONS, LICENSES, TRAINING, REGISTRATIONS:

TB Test, CPR, First Aid and SAMA to be provided within 30 days for hire.

TAC web-based training

Certified Community Behavioral Health Clinic Trainings completed within the first year of hire.
