The Certified Registered Nurse Practitioner (CRNP) will function in the Psychiatric Department within the Outpatient Program at Northern Tier Counseling, Inc. Adherence to professional CRNP standards will be maintained. The CRNP will work to implement clinical interventions that achieve client- focused goals which support Northern Tier Counseling’s vision for the delivery of excellent quality care. The CRNP will effectively interface with all medical department and clinical staff for the practice of coordinated clinical care and the demonstrated ability to work in the ever changing health care system with clinical competency and professional accountability. A Psych LPN is assigned to the position for assistance. (Psychiatric background and certification preferred)
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Work collaboratively with the staff psychiatrist and LPN in the practice of psychiatric assessment and diagnosis, implementation of psychiatric treatment interventions including evaluation and medication management: initiate a new psychiatric medication regimen, review psychiatric medication management interventions; provide written prescriptions and/or provide available medication samples, confirm client’s medication information at dispensing pharmacy and call in new medications and/or renewals, participate in individual and group medication management interventions and crisis management for the provision of comprehensive, holistic care.
2. Effective professional liaison with the client’s primary care physician as a representative of the Northern Tier Counseling’s Outpatient Department for effective communication and integrated client care. The CRNP, recognizes the need for on going communication and interface with hospital and community physicians and professionals to achieve the goal of continuity of client care.
3. A professional interface with the individual client to assure and protect the client’s confidentiality in all aspects of prescribed interventions.
4. Practices medical and psychiatric assessment skills for referral of mentally ill clients who need a more specialized or complex work up to a primary care physician and/or specialist. Provides on the spot health promotion and preventive services for medical problems with follow up referral for routine physical health screening and follow up by the PCP for identified health concerns.
5. Orders appropriate diagnostic tests related to the prescription of psychiatric medications and interprets test results for deviations from normal and document an appropriate plan of care in the client’s chart.
6. Develops and implements client clinical and medication management treatment plans, records all clinical interactions/interventions on the psychiatric case note and the medication record and provides clinical direction for effective continuity of care. Consistently and accurately documents all prescribed interventions.
7. Knowledgeable with and participates in the psychiatric hospitalization process (voluntary and involuntary) with effective assessment and collaborative documentation to support the hospitalization process. Accurate awareness of psychiatric advance directives and provides clear feedback to clients and support systems who indicate the need for advance directives to be part of the individual’s treatment plan.
8. With the appropriate release of information by the client, actively counsels and educates the client’s family and/or support system; when indicated provides brief psychotherapy interventions or makes referral for family counseling or physical and/or behavioral health care referral for a family member.
9. Affirms and counsels clients regarding compliance with a prescribed psychiatric regimen.
10. Recognition of medical emergencies and initiates appropriate first response interventions; CPR certified.
11. The CRNP will conduct medication check groups for the review of the client’s medications within the supportive and psycho educational group milieu. The CRNP, along with a Registered Nurse or Licensed Practical Nurse will lead the psychotherapeutic group exchange to review the client’s medication regimen and all related information connected to effective psychiatric medication management, prescribe and adjust medications when indicated, provide renewals and document all prescribed interventions.
12. The CRNP will effectively communicate with clients and their support systems as well as community professionals to address questions and concerns related to the client’s medication regimen and provide effective information for an informed decision regarding medication compliance.
13. The CRNP will provide identified psychotherapy based upon a blend of interpersonal theory and knowledge of short-term family models of care with an emphasis on time limited approaches while grounded in the latest neurobiology and psychopharmacology.
14. The CRNP will be expected to provide consultation services on an as needed basis for clients referred to the Community MH & D/A Center for a psychiatric evaluation while receiving clinical treatment in the community, clients at a skilled nursing facility, residential facility and correctional facility. Upon review with the staff psychiatrist, the consultation will provide the basic assessment information with specific guidelines for the PCP to follow or Northern Tier Counseling’s medication department will provide a brief period of prescribing psychiatric medications until stabilization is achieved and then the client will be returned to the PCP for on going care.
15. As required for all staff members of the Outpatient department, the CRNP is expected to respond to subpoenas with testifying at specific hearings for the purpose of clarification of the client’s psychiatric diagnosis, medication regimen, competency, etc
16. Participates in clinical program staff meetings, educational programs and is a clinical resource to all clinical staff by demonstration of professional role model skills. Actively participates in required supervision with staff psychiatrists. Demonstrates professional accountability for participation in continuing education seminars for required CME hours to maintain active license status and certification.
17. Professional assertive, interpersonal, organizational and effective communication skills with the essential skill to lead and work within a team environment.
18. CPR and other Continuing Education Requirements to maintain certification and active license required.
19. Works Closely with Outpatient Manager and Clinical Director to ensure clear communication and quality of care for patients.
20. Attends Manager / Supervisor meetings as needed.
21. Employee will be proficient in their job position within six (6) months. If at any time an employee feels they need more training/education, employee is to submit a request for such through their supervisor, manager, and/or director.
REQUIREMENTS:
1. Active Pennsylvania license and required certification information.
2. Master’s degree/Clinical Specialist with identified certification in specialty practice
3. Advance practice in the area of psychiatric nursing and/or behavioral health and addiction practices; all identified populations served in the community mental health and drug/alcohol clinic.
4. One to three years of CRNP experience.
5. Extensive ability to interpret policy, resolve problems with creativity and initiative and be an effective and willing participant in the change process

Pay: $70,000.00 - $80,000.00 per year
