Contracted Position

This agreement is made and entered into by and between the Southwestern Vermont Council on Aging (hereinafter referred to as SVCOA) and XX (hereinafter referred to as “RD”).
The purpose of this agreement is to utilize the skills of a contracted Registered Dietitian to enhance the quality of elder nutrition services supported by SVCOA in Rutland and Bennington County.
UNDER THIS AGREEMENT, THE CONTRACTOR WILL:
· Assist Nutrition Director and/or Nutrition Assistant with “nutrition education” presentation material/content to Rutland and Bennington County meal sites and senior housing units during the contract period.
· Review nutrition program menus to monitor quality and compliance with state standards and provide timely feedback to Nutrition Director.
· Contractor will be available for one-on-one counseling to individual elders who are considered “nutritionally at risk.”
1) Perform virtual nutritional assessment visits for clients in Rutland and Bennington County who have been identified as a significant nutrition risk.
2) Use the information gathered to develop a written nutritional care plan, including the use of nutritional supplements as appropriate. Provide a copy and explanation of the plan to the client and update the case manager.
3) Follow up via phone call or virtually with client(s) as needed.
- Provide in-service training and/or consultation to SVCOA case managers and other staff to help them identify clients who could benefit from nutrition intervention.
FOR ITS PART, SVCOA WILL:
- Reimburse the contractor for service and mileage at the following rates:
1) At an hourly rate of $40.00 with a cap of $10,000 for the contract period.
2) Reimburse the RD’s meal at the “guest fee” rate at the meal site.
3) The contractor will be reimbursed for mileage incurred in carrying out contract activities at the mileage reimbursement rate set by the SVCOA Board of Directors.
She will be notified of any changes in this rate during the contract period in writing.
CONFIDENTIALITY:
The contractor agrees to keep the information obtained from and about program participants confidential. It is understood, however, that appropriate information can be shared with authorized persons for the express purpose of serving the program participant.
BILLING/REPORTING/PAYMENT
Payment will be made quarterly, based on timely receipt of an invoice and/or activity report indicating hours worked and a reimbursement request indicating miles driven and meals eaten at meal sites during the month. Reports and reimbursement requests will be reviewed by SVCOA’s Nutrition Director who will authorize payment to the contractor for services provided. Payment will be made within ten days of receipt of these documents.
Qualifications
- Bachelor's degree in Nutrition, Dietetics, or related field.
- Registered Dietitian Nutritionist (RDN) certification.
- Proficiency in health coaching, patient assessment, and developing care plans.
- Strong knowledge of physiology and nutrition principles.
- Experience in tube feeding and diagnostic evaluation is preferred.
- Excellent communication skills and ability to work collaboratively in a healthcare team setting.
