```Responsibilities```
- Dispense prescription medications to patients and provide medication counseling
- Collaborate with healthcare professionals to develop and implement patient care plans
- Conduct patient assessments and monitor medication therapy outcomes
- Provide drug information to healthcare providers and patients
- Oversee the accuracy of medication orders and ensure compliance with legal and regulatory requirements
- Participate in medication reconciliation processes
- Perform IV infusion and compounding as needed
- Assist in medical management and utilization management activities
- Collaborate with the emergency department (ER) team to optimize patient care
```Requirements```
- Bachelor's or Doctor of Pharmacy (PharmD) degree from an accredited pharmacy program
- Active pharmacist license in the state of practice
- Strong knowledge of pharmacology, physiology, and acute care practices
- Completion of a pharmacy residency program preferred
- Experience in utilization review or utilization management is a plus
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a multidisciplinary healthcare team
- Strong attention to detail and accuracy in medication dispensing
