JOB SUMMARY
The Care Partner, as part of the direct service team, is responsible for assisting residents in maintaining their optimal level of functioning. He/she must understand and carry out the I’m Still Here™ philosophy of the Jewish Association on Aging and the Hearthstone Institute. The Care Partner must have a personal commitment to promote and advocate dignity, respect, individual rights, and a feeling of self-worth for each resident. The duties will encompass the areas of activities, assisting residents with their medication arrangement, meal services, and personal care, but will not be limited to just these areas. He/he must have the ability to work independently and cooperatively with other disciplines. Flexibility in adapting to the changing needs of the residents is essential.
JOB DUTIES / RESPONSIBILITIES
- Assist in maintaining a safe environment by being aware of hazards, and remove them when necessary.
- Must be knowledgeable about fire and disaster procedures and participate in monthly fire drills and/or emergency preparedness drills.
- Be alert to fire hazards and observe and enforce the facility’s no smoking policy.
- Assist with personal care of the residents using I’m Still Here™ techniques. This includes, but is not limited to, all ADL’s, bathing, dressing/undressing, oral hygiene, skin care, bladder and bowel function, bed making, and laundry. Dressing includes, but is not limited to, shoes, socks, tying shoe laces, fastening buttons, zippers, and Ted hose.
- Answer emergency lights promptly; assist with all emergencies as appropriate.
- Observe residents for physical, cognitive, behavioral, and social changes and report to supervisors immediately.
- Perform dining room duties in the resident dining room during meals utilizing the I’m Still Here™ approach to the dining experience. This includes but is not limited to, taking residents’ meal requests, serving, cutting food when needed, and opening items when needed, observation/supervision during the meal, and clearing tables after the meal. Assist with any medical emergencies that may arise during meal times and toilet residents as need during meal times. Assist with preparation of snacks or light meals for resident requests outside of regular meal times.
- Assist with light housekeeping chores such as laundry, and other light cleaning duties that are important to the residents’ health, safety and sense of well-being.
- Escort residents within the Weinberg Village campus or to any out of the building programs as needed.
- Communicate with residents in a simple, concise, adult and respectful manner using I’m Still Here™ techniques.
- Observe the policies and rules of the facility, as described in the Employee Handbook including, but not limited to, Dress Code, Conduct Code and Attendance Policies.
- Communicate with co-workers and supervisors via written and oral means (24-hour report, communication book) to ensure continuity of care.
- Promote, maintain and enhance each resident’s quality of life through socialization, communication, and participation in parallel programming and other I’m Still Here™ recreational techniques.
- Observe and communicate each resident’s whereabouts to team members when necessary.
- Respect and uphold the religious laws of the facility.
- Treat information regarding residents, families, and staff in confidential manner. Provide incontinence care to residents as needed.
- Participate in staff development programs such as attending scheduled in-services and annual in-service day.
REQUIRED EDUCATION, TRAINING AND EXPERIENCE
- High school diploma/GED or active status on PA nurse aide registry.
- Must be at least 18 years of age.
- Ability to read and write and to carry out instructions.
- Current CPR and First Aid Certification-will be provided within 30 days of hire.
REQUIRED SKILLS and ABILITIES
- Must be able to bend, push, pull and lift 50 pounds.
- Must be able to stand extensively and perform assigned duties.
- Must demonstrate sensitivity to, empathy for, and an interest in the elderly and those with cognitive impairments.
- Must embrace the I’m Still Here™ approach to resident care and show a successful track record of working with people with advanced cognitive impairment.
- Must be able to demonstrate consistent ability to work in a multi-disciplinary team environment.
- Must have the communication skills necessary to work with residents, families and colleagues as a team member.
- Must maintain positive working relationships with personnel from all departments.
- Must observe the basic principles of good nutrition, personal hygiene, and infection control when caring for residents.
- Must document properly those procedures done as part of assignment.
