Case Management Services Manager
Mazzoni Center’s Medical Case Management Program is a critical component of our services for people living with HIV/AIDS. Our program is staffed by qualified, compassionate professionals who dedicate themselves to serving the particular needs of each client. Recognizing that there are many factors that can impact health, we take a holistic approach to providing care services.
Responsibilities

Assist in overseeing the daily operations of the Medical Case Management program.

Provide weekly supervisions to Medical Case Managers and maintain written notes of all supervision.

Complete introductory and annual performance evaluation for supervisees.

Conduct chart reviews (hard files/Care Ware/athenaNet) for supervisees.

Assist with interviewing new staff for the Care Services Department.

Assist in the orientation and training of new staff.

Assist with client-related crises as necessary.

Complete monthly reporting.

Assist with Quality Management activities.

Coordinate and support the development of the Medical Case Management program by collaborating with outside agencies and professionals working in the field.
Qualifications

Master's Degree in social service (ie: social work, psychology, counseling, therapy)

Minimum of two years’ experience doing social work or medical case management.

Supervisory skills (with some supervisory experience).

Knowledge of HIV/AIDS and LGBTQ issues and community resources.

Demonstrated strong oral and written communication skills.

Demonstrated ability to prioritize work and manage multiple deadlines and responsibilities.

Ability to maintain client and staff confidentiality.
-
Bilingualism, Spanish/English, is a plus.
Physical Requirements:[1]

The ability to efficiently move around a city location on a regular basis.

The ability to communicate using written and verbal or signed language.

The ability to lift objects greater than 35 pounds.

The ability to use a computer for up to 6 hours per day.

The ability to work in an environment with a moderate noise level, in an open cubicle setting.

Must have all necessary vaccinations pursuant to Mazzoni Center policies.
