Job Title: ODR Case Manager
Department: Intensive Case Management Services/Office of Diversion and Reentry
Reports To: Program Manager
Under a contract with the Los Angeles Department of Health Services, Alcott Center a nonprofit community mental health agency has an immediate opening for organized, motivated, and professional candidates for FULL-TIME positions. The ODR Case Manager will travel to multiple locations in the greater Los Angeles area to meet and work with members to attain and sustain permanent housing in partnership with the Department of Health Services (DHS). The Case Manager’s primary responsibilities are to provide outreach and engagement to individuals referred through DHS from the Los Angeles County Jail system and to coordinate housing, mental and primary health, and recovery services that will ensure housing stability and community integration.

Essential Duties and Responsibilities include, but are not limited to the following:*
- In collaboration with DHS, the Sheriff Department, Probation, and DMH outreach to homeless individuals the jail system who have chronic health conditions and/or are high utilizers of jail and/or emergency county health services with the objective of engaging individual to determine eligibility for permanent housing
- Complete pre-authorization and application package to submit to DHS per their policies and procedures
- Assist engaged members with homeless rental subsidies and help then to find a safe place to live by contact Property Managers and Landlords to set up appointments to show members apartment options.
- Travel to multiple housing locations in the greater Los Angeles area to provide case management; coordinate and implement mental health and recovery support services with individual members (caseload of 15).
- Coordinate with DHS to ensure members are connected to primary health care and reduce need for emergency health care services
- Develop effective, trusting relationships with members, with a focus on facilitating independence and maintenance of improved physical and mental health
- Collaborate with each member to develop their individual service plan; review and update it quarterly and upon completion of goals.
- Develop and maintain a complete, accurate, and current member file with all required documents from DHS.
- Other duties may be assigned as needed
Qualifications:
- At least one year of experience working with homeless individuals AND a social work/mental health-related bachelor’s degree.
- Experience working with clients with mental illness, chronic health issues, and substance use disorders.
- Knowledge and expertise in the following areas: chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights, eviction prevention.
- Able to perform basic computer skills required for an electronic health record
Requirements
- Los Angeles Sheriff Department for jail clearance and proof of valid California Driver’s License and Live Scan
- Ability to maintain company vehicle insurability
- Must be fully vaccinated for COVID-19, including one booster
This is a full-time position with a 40-hour work week; the agency is currently open for services Monday through Friday, 8:30 am-5:00 pm.
