7am to 5pm Wednesday through Saturday $17.29/hour
7am to 5pm Sunday through Wednesday $17.29/hour
10pm to 8am $20.04/hour
LOCATION: Aurora, CO
The Care Navigator role is an excellent entry-level opportunity for anyone entering into the fields of human services, social work, or counseling/crisis support. The Mile High Behavioral Healthcare Housing Support Team (HoST) works across three sites in Aurora—the Comitis Crisis Center, Aurora Day Resource Center, and Colfax Community Network. All three sites provide supportive services for people experiencing homelessness and/or at risk of homelessness. Care Navigators play a key role in supporting guests in the Comitis emergency shelter program and the Aurora Day Resource Center (ADRC) daily programming—they are the first people who connect with clients and can support them with ongoing needs and referrals to other resources. The HoST sites provide a dynamic rapidly changing work environment, and Care Navigators work with high-need, high-intensity clients who are often experiencing acute crisis.
CARE NAVIGATOR PRIMARY DUTIES:
- Support guests with all basic needs services (site specific), including meals, mail, laundry, storage, showers, shelter beds, and hygiene items
- Support guests with referrals and resources—connections to internal HoST program and/or programs and resources offered by other agencies and organizations
- Support with initial intake into services including supporting with initial data collection survey for the Homeless Management Information System (HMIS)
- Answer phones and be available to assist as clients come in and out of the facility
- Support volunteers, Pioneer program participants, and donors on-site
- Ensure facility is a safe and clean environment including daily cleaning duties and active support with interpersonal dynamics in the milieu and de-escalation
- Gather and organize client related data, e.g. daily sign in information and program participation information
- Work with external agency partners to ensure that onsite programming runs smoothly
- Coordinate with partnering agencies to best serve clients during cold weather emergencies and will be cross-trained with our overnight shelter to provide these services as well
- Other duties as requested
CARE NAVIGATOR QUALIFICATIONS:
- Bachelor’s degree in human services, social work or counseling and/or equivalent professional experience preferred
- Experience working with people experiencing homelessness preferred
- Demonstrated ability to effectively work with people of all backgrounds
- Spanish-speaking a plus
CARE NAVIGATOR OTHER REQUIREMENTS:
- Understanding of behavioral health and homelessness services
- Compassion for the HoST guests and a connection to the mission
- Proof of automobile insurance and clean background check required
- Good rapport with the client population
- Demonstrated ability to constructively work with persons in crisis situations
- Must be able to work a flexible, non-traditional schedule including working additional cold weather emergency shelter shifts
- Any experience with harm reduction philosophies, trauma-informed care, and low-barrier shelter services, is extremely helpful
