The Nurse Practitioner (NP) collaborates with contracted community primary care physicians (CPCP) and the Interdisciplinary Team to ensure comprehensive oversight and coordination of participant care. The NP focuses on addressing functional, clinical, and psychosocial needs to promote participant independence and wellness.
Compensation: $75 - $85/hr
Location: New York, NY - 8:30am - 5:00pm
Responsibilities:
- Conduct annual wellness assessments, with more frequent evaluations for high-risk participants.
- Provide urgent care and manage acute changes in participant conditions.
- Perform post-hospitalization assessments within 5 days, including medication reconciliation and care coordination.
- Maintain accurate and comprehensive medical documentation, ensuring proper diagnosis coding.
- Participate in clinical documentation improvement activities and provide feedback to clinical staff.
- Collaborate with PCPs to manage high-risk participants, addressing care gaps and developing care plans.
- Ensure compliance with HIPAA regulations and maintain the security of Protected Health Information.
- Supervise the care plan formulation and delivery for community PCP participants.
- Oversee home care services arranged by Community Health Nurses (CHN), ensuring appropriate and effective care coordination.
Qualifications:
Education:
- Graduate from a nurse practitioner education program or certified as a nurse practitioner by a national certifying organization.
Experience:
- Minimum of 3 years in clinical nursing practice in-home care, care management, or a related field.
- Strong oral and written communication and interpersonal skills.
- High-level analytical and research skills.
- Ability to manage multiple projects and meet deadlines.
- Proficient in Microsoft Office.
Licenses and Certifications:
- Licensed to practice as a Nurse Practitioner in NY.
- DEA Certification.
- AANP or ANCC Board Certification.
