Two + years experience in clinical or community resource settings; care coordination experience is desirable.

The Neurosurgical Patient Navigator works in collaboration and continuous partnership with chronically ill or high risk patients (Neuro-Oncology, Neurosurgery, and/or those with a diagnosis of a brain disease) who are not followed at a SKCC Multi-Disciplinary Clinic (MDC) but who are followed by the Vickie and Jack Farber Institute for Neuroscience at Jefferson in Center City.
S/he will coordinate care between diagnosis, treatment, survivorship or end of life. He/she participates in the assessment and ongoing support of patients in close collaboration with the the Care Team(s) s/he is supporting.
ESSENTIAL FUNCTIONS:
- Serves as the point of contact, advocate, and informational resource for patients with a non-oncology diagnosis,their care team, & family/caregiver(s).

Promotes timely access to appropriate care such as helping to schedule appointments from first call to first treatment.

Gathers records, organizes and supports care team.

Monitors and promotes adherence to a care plan, developed in coordination with the patient, PCP, specialist and family/caregiver(s)

Supports communication between the care team and the patient, facilitates screening and addresses barriers to care and documents all interactions in the patient’s electronic record

Connects patients to relevant community resources, with goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs

Transistion general patient navigation services to SKCC oncology patient navigator if/when patient requires MDC services.

Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson.
