- Perform direct patient care to high need (trach, vent, g tube etc) pediatric and adult clients in their homes
- Responsible for documenting and providing skilled nursing care to our patients, following the developed plan of care and the patient’s physician orders for each of our patients; while adhering to confidentiality and professional boundaries.
- Ensure quality and safe delivery of home health care services by utilizing basic nursing skills and the nursing process.
- Recognize and institute when emergency measures are needed in a rapid, appropriate manner
- Participate in the coordination of home health services, appropriately porting the identified needs for other disciplines (i.e. PT, OT, ST) to the DON or RN supervisor
- Providing care according to the patient’s plan of care, assistance with daily tasks, which include wound care, injections, assessments, vitals, glucose checks, blood drawing, treatment plans, record keeping, overseeing CNA/PCA, med passes, etc.
