QUALIFICATIONS:
- Current NYS license as a Registered Professional Nurse
- Two (2) years experience as a Registered Professional Nurse with administrative/supervisory experience; or
- Completion of a Baccalaureate degree in Nursing; or
- Completion of a Master’s degree Program in Nursing, including public health and/or administrative studies; or
- Public Health nursing certification.
- Must have Homecare experience.
- UAS Certification preferred.
RESPONSIBILITIES:
- Ensures compliance with federal, state, and local DOH regulations.
- Ensures compliance with Joint Commission standards.
- Supervises Field Nurse in relation to assessment of patient Health Care service needs regularly to ascertain level and degree of patient needs, especially when a change in patient’s condition or situation occurs and provides clinical consultation; problem analyses, and problem solving.
- Ensures clinical compliance where People Care incurs clinical responsibility.
- Administers (in conjunction with the CDCS&QM) professional clinical staff competencies as needed.
- Initial intake and compliance oversight for all new HRA cases.
- Schedule in-home visits to direct, demonstrate and evaluate the delivery of patient care as appropriate.
- Responsible for clinical oversight of all case management of HRA/MLTC cases and UAS Review as needed. Performs on-going clinical record review to ensure compliance.
- Ensures that the patient’s clinical record is revised related to patient status and response to plan of care, changes in patient condition, informal supports or environment, adverse reactions, or discharge.
- Works with clinical team to maintain and review schedules of all Field Nursing Staff. Assigns monthly visit roster to all field nurses, and monitors for timely completion of assessments, supervisions, and ongoing MD orders.
- Serves as liaison between patient, patient advocate, and other home care team members and referral to outside agencies regarding patient service needs.
- Documents interim contacts with physician, patient advocate, interdisciplinary team members, and outside agencies regarding patient service needs.
- Ensures availability of services and appropriate staffing to promote quality of care.
- Ensures that clinical incident reports for HRA are completed and followed up according to contract and company guidelines.
- Assists as needed to maintain patient and employee exposure logs, infection control logs, occupational exposure logs, and special circumstance log.
- Ensures review of pre-employment and annual physicals for compliance with Agency Policy and Procedures, NYS DOH and Joint Commission Standards.
- Administers PPD’s when necessary.
- Chairperson for the HRA Performance Improvement Committee, coordinates record reviews and data collection. Works with DCS and CDCS to prepare for ongoing Corporate P.I. meetings.
- Provides Agency representation on various advisory boards and health agencies.
- Conducts annual performance reviews of all nursing department staff.
- Assists the CDCS&QM in policy formulation, revision, and enactment.
- Performs case review with paraprofessional coordinators, when necessary.
- Assumes clinical on-call responsibility for the branch office.
- Abides by the policies and procedures of the agency.
- Communicates well with DCS, and assists with coverage in the absence of DCS.
- Performs related duties as assigned.
HEALTH REQUIREMENTS:
- A completed People Care, Inc. physical form or acceptable substitute containing required information.
- Two step PPD (Mantoux) skin test for tuberculosis prior to employment and no less than every year and thereafter for negative findings. Positive findings shall require appropriate clinical follow-up but no repeat skin test.
- Proof of Immunization to Rubella.
- Proof of Immunization to Rubeola, if applicable.
- A negative drug screen.
- Any test that may be required by the local Board of Health.
- An annual, or more frequent in necessary, health assessment.
WORK ENVIRONMENT:
Regular exposure to office equipment and technology i.e. telephones, computers, business machines etc. Occasional stress.
COGNITIVE REQUIREMENTS:
Able to communicate clearly, verbally, and in writing. Able to follow directions given verbally and in writing. Able to integrate multi-sensory information in the course of completing everyday job duties. Able to direct assigned staff in their daily responsibilities.

Pay: $85,000.00 - $95,000.00 per year
