Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing comprehensive support and guidance to individuals in need of behavioral health services. Your role will involve assessing patient needs, developing treatment plans, coordinating care, and advocating for the well-being of your clients. Responsibilities:
- Conduct patient assessments to determine their needs and develop individualized treatment plans
- Collaborate with healthcare professionals to coordinate and monitor patient care
- Provide addiction counseling and support to individuals struggling with substance abuse
- Utilize utilization management techniques to ensure appropriate use of resources
- Assist patients with discharge planning and transition to appropriate levels of care
- Advocate for patients' rights and ensure they receive necessary services
- Maintain accurate and up-to-date documentation of patient interactions and progress
- Collaborate with social workers, therapists, and other professionals to provide holistic care
Requirements:
- Bachelor's degree in social work, psychology, or related field (Master's degree preferred)
- Proven experience in case management or related field
- Strong knowledge of addiction counseling, behavioral health, and acute care services
- Familiarity with utilization management techniques and discharge planning processes
- Excellent communication and interpersonal skills
- Ability to assess patient needs and develop appropriate treatment plans
- Proficient in documentation and record keeping
