The Youth Residential Therapist ("Therapist") is an integral member of Community Reach Center's youth shelter services which provides short-term shelter services for youth 12-18. The Therapistprovides clinical and support services to consumer and/or their families through integrated behavioral healthcare services. The Therapist role is a hybrid role, providing all Care Management and Therapy needs to a 10 bed youth shelter. The Therapistapproach will utilize a Psychosocial Rehabilitation Model that emphasizes assisting persons to explore and develop social, occupational, leisure, and living skills, including family reunification and interagency cross-referrals.
The Therapist provides clinical and support services to consumers currently enrolled in the Youth Shelter, Empowerment Center for Adolescents ("ECA"). This is including but not limited to individual therapy, group therapy and family therapy working on family reunification.
Additionally, the Therapistwill have other duties and responsibilities as determined by the Program Manager.
Essential Functions:
- Provides prevention/early intervention, case management, family psychoeducation, and other clinical interventions to consumers and families as appropriate to the position.
- Works directly with Department of Human Services on service plans, care coordination, etc, when indicated
- Provides direct individual and group therapy services to current ECA clients, utilizing brief evidence-based, strengths-based, and person-centered interventions.
- Completes comprehensive intake assessment, including diagnostic interviewing, and develops/maintains personalized treatment plan.
- Coordinates with internal and external treatment teams and community partners to conduct collaborative discharge planning.
- Crisis intervention/de-escalation as needed.
- Track client follow-up and clinical outcomes using Electronic Health Record system (EHR). Document in-person and telephone encounters in EHR.
- Attend collaborative treatment team meetings and reviews of consumers.
- Assist eligible consumers in gaining access to needed medical, social, educational, and other services.
- Provide an assessment of needs, service plan development, referrals/linkage, and monitoring/follow ups for consumers.
- Attends and participates in in-service education and training opportunities, meetings, clinical supervision, and special events; utilizes opportunities that provide professional growth.
- Facilitates and contributes to the accomplishment of tasks by consumers, empowers consumers to reach optimum level of achievement, and sets a positive example and provides role modeling as well as technical expertise to accomplish goals.
- Provide transportation for consumers in agency vehicles when clinically indicated.
- Will work closely with outside organizations on resources and care coordination.
- Uses identified agency key performance indicators to track success, engagement, and other factors leading to successful completion of treatment.
- Understands and works within state regulations required for program licensure of Homeless Youth Shelter and Residential Child Care Facility.
Qualifications:
- Required: Master's Degree or higher Mental Health related field
- Clinical License (LPC, LP, LCSW, LMFT) preferred, eligibility for licensure (LPCC, PSYC, SWC/LSW, MFTC) required
- Knowledge and understanding of clinical social work and/or mental health counseling principles, methods, procedure, and ethical standards required
- Familiarity with current DSM 5 and diagnostic techniques required
- Ability to work independently in a residential setting.
- Valid Colorado Driver's license with a clean driving record.
- 6 months of experience working with youth and shelter services preferred but not required
- Bilingual Spanish preferred, but not required
Physical Requirements:
- Regularly required to, stand, sit; talk, hear, and use hands and fingers to operate a computer and keyboard.
- Specific vision abilities required by this job include close vision requirements due to computer work.
- Light to moderate lifting is required,
- Regular, predictable attendance is required.
Schedule:
Full time 40 hours per week. Some days 8am-5pm, At least 2 days per week 10am-7pm. Can be flexible with these days.
Salary Information:
Starting Salary Range: $57,000 to $65,000 annually.
Salary Range: $57,000-$72,200 annually
Eligible for $250/month bilingual stipend bonus
