Senior Case Management Services on the North Side of Chicago provides comprehensive services so that seniors are able to live independently or with caregivers in their own homes and communities. We offer a wide variety of programs and services for older adults and their family members. Coordinators provide information, assessments and connect clients to services such as, homemaker, adult day care, emergency home response, caregiver assistance, and transportation, Meals on Wheels, and benefits assistance. Community Outreach presentations about our programs and services are also available.
Senior Care Coordinator I-Service Coordination/Care Coordination Unit
- Home visits are made by trained Care Coordinator I’s to assess physical, cognitive, social and financial needs and discuss care options. The Care Coordinator then develops and monitors plan of care. Will complete Service Coordination responsibilities for residents in a senior building part of the week and will conduct home visits for Comprehension Care Coordination of seniors living in the community the other part of the week. May work in a hospital, agency site, and/or conduct home visits.

- Preferred Requirements: Bachelor’s degree. Three years of relevant experience. Illinois Department of Aging Certification.
- Minimum Requirements: Bachelor’s degree. Illinois Department of Aging Certification upon hire.
- Other Requirements: Required to travel throughout the assigned communities to complete job duties. Driver’s license.
*Responsibilities *
- Works independently and consistently exercises discretion and sound judgment in engaging and assessing the complex needs of elderly or frail clients by gathering information and developing their care plans. May work in a hospital, agency site or conduct home visits.
- Conducts intake assessments for new program participants within prescribed time frame and provides case management services, by informing clients and their families of the benefits and services they may qualify for, and assists in obtaining such services. Completes necessary documentation for program services in the prescribed time frame. Responds to clients, authorized representatives, collateral contacts and other contacts within 24 hours.
- Creates well written and effective care plans which appropriately match the needs of clients following the standards and guidelines of the funders, contractors and governmental regulations.
- Makes appropriate referrals, monitors client services in the appropriate time frames, seeks supervisory guidance as required, documents services provided and completes billing as prescribed by the department.
- Utilizes community resources and regularly contributes updates to these resources so that others might use them as well.
- Maintains up-to-date and accurate client files and databases, as required by agency/programmatic policies and procedures, professional standards, and external requirements of all relevant regulatory, licensing or accrediting bodies.
Participates in program meetings and individual and group outreach and education
