Title: Nurse Practitioner – Spanish Bilingual
Provides transitional care management for patients via electronic devices in a telehealth setting (remote)
Experience: Must have a NPI number and PTAN number with a minimum of 1-2 years of direct experience in a Nurse Practitioner role; and active CA License.
*2-5 years experience as a licensed nurse in the following settings preferred: ER, ICU, Telemetry, Geriatrics, or Primary Care.
Direct responsibilities: include the following and other duties as assigned:
- Ability to communicate fluently with English and Spanish-speaking medical patients
- Review hospital medical records prior to first visit to understand patients’ condition.
- Explain our program to patients and caregivers on the first call
- Reviews and discusses patient’s current condition and concerns
- Review discharge plan with patients to ensure patient understanding and compliance
- Monitors effectiveness of interventions and discharge plan and remediates any deficiencies in discharge plan and patient care
- Conducts medication reconciliation, patient education, and monitors compliance, ensures patient scripts have been filled and patient has adequate supply
- Informs patient of safety concerns and warning signs of condition changes
- Informs steps to take, who to call if condition changes or patient has concerns
- Notes significant changes in patient’s condition and informs patient PCP or supervising physician
- Completes all pre and post call documentation
- Notes complexity of medical decision making in documentation
Minimum qualifications:
- Graduate of an accredited school of nursing
- Graduate of an accredited Nurse Practitioner Program
- Current California nursing license
- Advanced Practice Registered Nurse (APRN) preferred
- Strong computer skills
- Excellent phone communication and interpersonal skills
Job Types: Full-time, Per diem
Pay: $60.00 - $80.00 per hour
Expected hours: 25 – 40 per week
