Position Title: Home Based Crisis Intervention Counselor
Position Summary:
The Counselor under the supervision of the Coordinator, include but not limited to counseling and home visits to clients, escorts services for clients and attending team meetings and charting of all notes. Counselor must have a Master’s Degree or Bachelor’s in Social Work or a related field with social service experience.
Major Duties:
- Intakes for a minimum of 3 unduplicated families per month.
- Consent and complete intakes within 2 business days
- Assist counselors in the field for initial face to face visit.
- Provide crisis intervention services to families at any time
- Function in a crisis-oriented 24 Hours per day ‘On Call’ environment
- Intake assessment, consultation, and direct services
- Provide case management services
- Participate in interdisciplinary team meetings.
- Up-to-date records of each treatment sessions, implementation of treatment plan, discharge, etc.
- Responsible for caseload and maintaining program levels of service.
- Complete progress notes and chart documentation in a timely manner.
- Participation in administrative meetings, conference, and in-servicing training
- Other duties as assigned by supervisor or director.
Qualifications:
- Master’s Degree in Human Services or Social Work preferred or BA/BS/BSW degree with 4 years of experience in social services
- Interest in working with children/adolescents with mental illness.
- Exceptional communication and organizational skills, including excellent phone demeanor and direct communication skills with program participants and providers
- Ability to operate with purpose, urgency, and accuracy in a fast-paced deadline-driven environment;
- Must maintain confidentiality and have the ability to exercise a high level of judgment/discretion
- Knowledge and proficiency of MS Word, Excel, PowerPoint, and possess a willingness to lean new programs as needed;
- Must be able to work independently and manage multiple tasks in a fast-paced environment;
- Ability to work collaboratively with a variety of people, as well as an aptitude for teamwork;
- Ability to use public transportation for travel to program locations throughout New York City.
- Bilingual English/Spanish.

Position requires fine moto repetitive motion and the ability to make skillful, controlled manipulations of small objects. Physical agility to bend, stoop, walk, reach overhead, push/pull, squat, twist, and turn. Physical ability to remain on feet for extended periods throughout a workweek. Regular crouching to retrieve items from the floor or lower shelves. Regular reaching to retrieve items form storage shelves and file cabinets up to 8’ high. The employee must frequently lift and/or move heavy and/or bulky objects weighing more than 50 lbs. Must be physically capable of performing above tasks.
